Purpose: To investigate the relationship betweem knee pain and running posture and the effect of using the half foot insole among female runners.Methods: 17 female runners with knee pain and 20 healthy female runners were recruited into this experiment and divided into Pain group and Normal group.Participants were instructed to run 10 kilometers for 2 times with and without insole,and kinematics,foot posture index,pain,RPE were tested before and after running.The interval is a week.Results: 1.Before 10 kilometers running,participants in Pain group showed no significant difference(P> 0.05)with Normal group in kinematics.After 10 kilometers running,participants in Pain group showed greater pelvic drop angle than Normal group with significant difference(P< 0.05).2.Comparing the change before and after 10 kilometers' running,participants in Normal group showed decreased trunk forward leaning angle(P<0.01)and knee flexion angle(P<0.05).Participants in Pain group showed increased pelvic drop angle(P<0.05).3.After 10 kilometers' running with half foot insole,participants in Normal group showed decreased ankle dorsiflexion angle with significant difference(P<0.05),and no difference in trunk forward leaning angle and knee flexion angle(P>0.05)comparing with test before the first 10 km running.Participants in Pain group showed significant decreased pain VAS score(P<0.01),and pain occurred significant further in distance(P<0.01).And participants showed decreased ankle dorsiflexion angle with significant changes(P<0.05),and no difference in pelvic drop angle(P>0.05).4.After removing the half foot insoles immediately,participants in both groups showd no significant changes(P>0.05)with the test before first 10 kilometers running.5.Foot posture index increased in two groups but with no significant difference(P>0.05).There was no significant difference between groups before and after wearing half foot insole in foot posture index and RPE(P>0.05).Conclusions: 1.The lateral stability of trunk and pelvic in femalerunners with knee injury is weaker than healthy female runners,which could be a risk facator of injuries.2.Half foot insole could support the foot arch of runners with knee pain,to decrease the lateral instability of pevils through alignment change of lower extremities due to fatigue,and help mantain running posture while managing pain.Half foot insole can help healthy female runners maintain a more stable running posture to prevent injury potentially.3.The delayed effect of half foot insole is not existed with immediately wearing.
