As an important part of the social security system,social assistance is the basic institutional arrangement established by the state to guarantee the right to life and health of all citizens.Since the Eighteenth National Congress,the state has been highly concerned with "poverty caused by illness" and "poverty returned due to illness".Through the introduction and implementation of a series of policies,it has now opened up a good situation for the needy masses to "have a cure for illness".However,there are still a large number of low and middle-income rural residents who can not afford to pay for their own medical expenses and are in a dilemma of survival.The Marginal Poverty Problem of the sandwich group of social assistance,namely "poverty due to illness expenditure",seriously threatens the achievement of the goal of "building a well-off society in an all-round way",and has become a social issue of close concern to all walks of life.Zhengzhou,as the central city of the Central Plains urban agglomeration,has the typical characteristics of the Central Plains city with dense population and unequal regional economic development.Research on the experiences and issues of expenditure-based poverty caused by illness and expenditure in rural areas of Zhengzhou can bring some enlightenment to the related system construction of other Central Plains cities.Guided by the theory of social needs and welfare pluralism,this study used literature analysis and case analysis to study the situation of poverty-stricken families in rural areas of Zhengzhou City,Henan Province.Generally speaking,in Zhengzhou,the rural poverty-stricken family assistance based on expenditure for illness has some problems,such as the lack of popularization of relevant policies,narrow scope of assistance,poor service quality of designated medical institutions,etc.The sustainability of the development needs assistance for the recipient families,such as the loss of employment opportunities of the working population and the worrying psychological state of family members and absence of attention to livelihood needs,etc.According to the limited effect of the current life assistance,for poor families with expenditure on illness,we find that the reasons for the supply of survivable social assistance are: the related functions of government departments are not playing well,the institutionalized relief policy has not yet landed,the consciousness of public medical institutions to improve themselves is poor.And the reasons for the supply of developmental social assistance are: the lagging of the concept of relief,lacking of the participation of social forces.Therefore,it takes forward measures to optimize the allocation of functions of management institutions,improve the efficiency of relief services,give full play to the social functions of the government,speed up the implementation of institutionalized policy landing,reform of "supervision + payment mode",improve the level of medical services and other measures to guarantee the survival needs of families in need,and reform the concept of social assistance,enhance the growth needs of care groups.In the meanwhile enrich medical institutions resources,alleviating the predicament of family developmental needs of patients,introducing social work services,improving the psychological status of demand groups,promoting community participation in related assistance,creating a harmonious living environment for the poor,and other measures advocating multi-forces to work together to meet the developmental needs of poor families in order to build a more practical and comprehensive protection of the rural poverty-stricken family relief system based on expenditure on illness to increase strength.
