Objective: Mechanical heart valve replacement(mechanical heart valve replacement,MHVR)has been shown to be a fairly safe and effective procedure.The incidence of valvular heart disease is increasing year by year worldwide with the change of people's life style,and more and more young patients choose the mechanical heart valve replacement.However,due to valve noise,long-term use of anticoagulant drugs and other factors,MHVR patients' quality of life level decreased.In order to improve the quality of life of MHVR patients,therapeutic communication system(TCS)was applied to MHVR patients in this study,aiming to evaluate the intervention effect of therapeutic communication system in MHVR patients.Methods :From April to July 2015,90 patients with mechanical valve replacement who visited the cardiac surgery department of Anhui Huainan Chaoyang Hospital were selected by convenient sampling method,and then divided into intervention group(45 cases)and control group(45 cases).Patients in the control group were treated with conventional health education methods for nursing intervention,and those in the intervention group were treated with TCS mode on the basis of the control group.The Chinese version of the SF-36 Health Inventory,the Hospital Anxiety and Depression Scale(HADS),and the Morisky Medication Adherence Scale(MMAS-8)were used to evaluate the patient's life quality,Anxiety and Depression,and Adherence to Medication at the baseline,one month after the intervention,and at three time points three months after the intervention.Results 1.There was no statistically significant difference in the total scores of SF-36 and scores of various factors between the two groups before and 1 month after the intervention(all P>0.05)between the two groups.3 months after intervention,intervention group in total score and the scores of the four factors(physiological function(PF),Physiological functions(RP),The emotional function(RE)and mental health(MH))were higher than those of the control group,and the difference was statistically significant(P<0.05).2.The depression scores increased(P<0.05)and the anxiety levels increased(P<0.05)in the control group after intervention.The depression scores were reduced(P<0.05)and the anxiety levels were reduced(P<0.05)in the intervention group after intervention.3.There was no significant difference in adherence to medications between the two groups at 1 month after intervention(P>0.05).However,the adherence to medications of the patients in the intervention group was significantly improved at 3 months after intervention.The difference with the control group was statistically significant(P<0.01).Conclusion: 1.The intervention of therapeutic communication system for MHVR patients can improve the he scores of the four factors,physiological function(PF),Physiological functions(RP),The emotional function(RE)and mental health(MH).2.The intervention of therapeutic communication system for MHVR patients can reduce the scores of anxiety and depression.3.The intervention of therapeutic communication system for MHVR patients can improve the adherence to medications.
