Objective:To explore the value of serum direct bilirubin(DBIL)in type 2 diabetes to predict diabetic nephropathyMethods:A total of 388 patients with type 2 diabetes who were hospitalized in the Endocrinology Department of the First Affiliated Hospital of Jinan University from January 2018 to January 2019 were selected.Divided into four groups according to the DBIL quartile: Group 1(0.3~2.1 ?mol/l),Group 2(2.1~2.8 ?mol/l),Group 3(2.8~3.8 ?mol/l),Group 4(3.8 ~6.8?mol/l).Measure the height and weight of patients with type 2 diabetes and calculate BMI(=weight kg/height 2m2).Subjects were allowed to rest for 10 minutes in a quiet environment,taking a sitting position to measure and record systolic/diastolic pressure.All subjects underwent fasting venous blood in the morning after fasting for 8 hours,and the aspartate aminotransferase,alanine aminotransferase,serum creatinine,total cholesterol,triglyceride,low density lipoprotein,high density lipoprotein,Erythrocyte sedimentation rate,fasting blood glucose,etc.Glycated hemoglobin was determined by high pressure liquid method,and urine microalbumin was measured by radioimmunoassay,and the ratio of urine microalbumin to urine creatinine was calculated.Results:Pearson correlation analysis of DBIL and blood biochemical indicators in patients with type 2 diabetes showed that DBIL was positively correlated with alanine aminotransferase(P<0.05),and was related to erythrocyte sedimentation rate,course of disease,cholesterol,low-density lipoprotein,triglyceride,and urine trace The albuminuria creatinine ratio(UACR)was negatively correlated(P<0.05).Multiple stepwise regression analysis of the influencing factors of UACR,the results show that DBIL,TC and UACR are related [B =-16.186,11.941,SE =4.317,5.281,? =-0.227,0.137,P <0.05],the regression equation is UACR = DBIL×(-16.186)+TC×11.941.The results of a binary logistic regression analysis of the factors affecting urine microalbumin showed that with the increase of DBIL,the risk of urine microalbumin decreased.The OR value was 0.304.DBIL was an independent influencing factor for urine microalbumin.Conclusion:As the level of DBIL increases within the physiological range,UACR decreases,and DBIL can delay the progression of diabetic nephropathy,which may reduce the risk of diabetic nephropathy...
