Objective: To investigate the correlation between serum total bilirubin and complications of type 2 diabetes mellitus. Methods: From October 2017 to October 2018,488 patients with T2 DM were treated in the Department of Endocrinology, the First Affiliated Hospital of Xinjiang Medical University.First, they were divided into lower extremity atherosclerosis group(301 cases)and lower extremity arteries based on color Doppler ultrasound results. In the normal group(187 cases),the clinical data and biochemical characteristics of the two groups were analyzed, and then the correlation between TBIL and related clinical indicators was analyzed. Finally, binary logistic analysis was performed to explore the risk factors of lower extremity arteriosclerosis.Second, the quartiles of TBIL concentration were divided into four groups to compare the incidence of diabetic retinopathy(DR),diabetic peripheral neuropathy(DPN),and carotid sclerosis among patients between the four groups. Results: The levels TBIL,direct bilirubin, indirect bilirubin, uric acid and γ-GT in lower extremity atherosclerosis group were lower than those in lower extremity arterial normal group, but the age and HbA1 c were higher than those in lower extremity arterial normal group(P<0.05).Pearson analysis showed that TBIL was positively correlated with DBIL(r = 0.557),IDBIL(r =0.740),creatinine Scr(r = 0.124),and UA(r = 0.090),and was related to age(r =-0.267)was negatively correlated(P <0.01).Regression analysis showed that TBIL(OR = 0.533,95% CI = 0.467-0.607)was a protective factor for lower limb atherosclerosis, and age(OR = 1.084,95% CI = 1.052-1.116)was lower limb atherosclerotic Risk factors. At the same time, the comparison according to the serum TBIL interquartile range showed that the incidence of DR,DPN,and carotid atherosclerosis gradually decreased in the low bilirubin group to the high bilirubin group. Conclusion: TBIL level is a protective factor for type 2 diabetes with lower extremity arteriosclerosis.Within a certain range,TBIL levels were negatively correlated with the incidence of DR,carotid sclerosis, andDPN.TBIL was negatively correlated with age and positively correlated with Scr and UA. |
