ObjectiveThrough to the knee arthroplasty patients with early eating for three days after the ginger apply umbilical,observed within 72 h of patients with gastrointestinal symptoms,anti-nausea drug use,postoperative exhaust defecation time for the first time,the change of bowel sounds,and comfort,evaluation of ginger apply umbilical prevention early eating after knee replacement in patients with gastrointestinal adverse effects.It provide clinical and theoretical basis for the treatment of such patients and provides simple and feasible nursing methods for the clinic.MethodThe patients who were admitted to the orthopedic ward of a third-grade hospital from November 2019 to August 2020 and underwent unilateral knee replacement under general anesthesia were studied.According to the inclusion and exclusion criteria,a total of 90 patients were included.According to the random number table method,the patients were divided into the experimental group and the control group,with 45 patients in each group,and1 patient in each group with a shedding rate of 2.22%.Finally,44 patients were included in the experimental group and the control group,respectively.Patients in the control group were treated with conventional treatment and placebo umbilicus application,while patients in the experimental group were treated with ginger paste on the basis of conventional treatment.The duration of umbilicus application was 7 o 'clock in the morning of the operation,15 min after returning to the ward,8 a.m.on the first day after the operation,and 8 a.m.on the second day after the operation.The duration of application was 6 hours each time,and a total of 4interventions were performed for 3 days.Gastrointestinal symptoms,the use of antiemetics,the duration of first postoperative defecation and exhaust,changes in postoperative bowel sounds,and patient comfort were recorded in the two groups within 72 h after surgery.The collected data were analyzed by statistical software SPSS22.0,and conclusions were finally drawn.Result1.Baseline comparisonThere was no significant difference in the general condition(age,gender,education level,height,weight,defecation,liver and kidney function,duration of operation,duration of anesthesia recovery,preoperative gastrointestinal symptoms and bowel sounds)between groups before intervention(P>0.05).It showed that the two groups were comparable.2.Gastrointestinal reaction evaluation within 72hThe main symptoms of gastrointestinal adverse reactions in the two groups were nausea and vomiting,decreased defecation,and hard feces within 72 hours after surgery,with less cases including abdominal ringing and belching,while the rest did not occur.This condition was considered to have occurred if the gastrointestinal symptom checklist score was ?2points.Compared with the control group,the incidence of nausea and vomiting,reduced defecation and hard feces were significantly reduced in the experimental group,with statistically significant differences(P<0.05).In terms of nausea and vomiting,the incidence of nausea and vomiting in the experimental group was significantly lower than that in the control group immediately after surgery,2h,4h,6h and 72 h,with statistically significant difference(P<0.05).There was no statistically significant difference in the incidence of nausea and vomiting between the experimental group and the control group 24 h and 48 h after surgery(P>0.05).The degree of nausea and vomiting was lower in the experimental group than in the control group,and the difference was statistically significant(P<0.05).In terms of defecation reduction,the experimental group was superior to the control group only in terms of incidence,but the improvement was not significant in terms of severity,and there was no statistical significance between the two groups(P>0.05).3.Anti-nausea drugsIn terms of the amount of antiemetic drugs used between the two groups,the experimental group was less than the control group,with statistically significant difference(P<0.05).4.First postoperative exhaust durationThe mean time of postoperative first exhaust in the experimental group was shorter than that in the control group,and the difference between the two groups was statistically significant(P<0.05).5.The length of the first postoperative bowel movementThere was no statistically significant difference between the experimental group and the control group in the average length of the first postoperative bowel movement(P>0.05).6.Changes of bowel sounds after operationThe frequency of bowel sounds per minute in the experimental group immediately after surgery and 6h after surgery was higher than that in the control group,the difference between the two groups was statistically significant(P<0.05).There was no significant difference in the number of bowel sounds per minute after 24 h between the two groups(P>0.05).7.Comfort levelThe immediate comfort level of the patients in the experimental group was higher than that of the control group 6h,24 h and 48 h after application of ginger umbilicus,and the difference in the comfort level of the patients in these three moments was statistically significant(P<0.05).As for the overall comfort level,the physical and psychological comfort level of the experimental group was higher than that of the control group,and the difference between the two groups was statistically significant(P<0.05);in terms of social culture and environment,the difference between the two groups was not statistically significant(P>0.05).8.SecurityDuring the intervention,no skin allergy related to ginger umbilicus was observed.Conclusion1.Ginger umbilicus application can effectively reduce the incidence of nausea and vomiting within 72 hours after TKA patients who eat early,alleviate symptoms,and reduce the use of antiemetic drugs.2.Ginger umbilicus application can promote the recovery of bowel sounds in patients who eat early after TKA,shorten the first postoperative exhaust time,and promotegastrointestinal peristalsis to a certain extent,but it does not significantly improve postoperative defecation.3.Ginger umbilicus application group can improve the physiological and psychological comfort of patients who eat early after total knee arthroplasty.
