Objective: The loss of life ability of patients after stroke is mostly caused by the dysfunction of upper limbs,especially hands.Hand functional exercise is the premise of alleviating hand dysfunction,and the relief of hand spasm is the basis of timely and effective hand functional exercise.Through preliminary clinical observation,our group found that Fengshi fascial point acupuncture can effectively alleviate hand spasm immediately after stroke,but the evidence needs to be supported by further large sample studies.Therefore,the purpose of this study is to observe the effect of Feng's fascial point acupuncture on hand spasm after stroke,to further evaluate its clinical effectiveness,and to preliminarily analyze and explore its mechanism of action.Methods: A total of 132 stroke patients who met the inclusion criteria were randomly divided into "Feng fascia point" acupuncture group,sham acupuncture group and control group according to the random number table method,with 44 cases in each group.The acupuncture group was dealt with acupuncture of "Feng fascia point" combined with routine rehabilitation treatment.After developing needle sensation,retaining the needle was for 30 minutes each time,and it was applied 5 times a week for 4 weeks.The sham acupuncture group was given acupuncture at the point 1.0 cm right from "Feng fascia point" 30 minutes a day for 5 days a week for 4 weeks and combined with routine rehabilitation treatment.The control group received routine rehabilitation.5 times a week for 4 weeks.The control group received routine rehabilitation therapy 5 times a week for 4 weeks.Prior to treatment,the modified Ashworth scale was used to assess the degree of spasm,and again after the first treatment and at the end of the treatment,and the duration of spasm relief was recorded.Carroll Hand Function Assessment was used to assess the hand function of patients,and modified Barthel Index Scale was used to measure the daily living ability of patients.Results: There was no significant difference in the improved Ashworth scale score,Carroll hand function score and daily living ability score among the three groups before treatment(P>0.05).The improved Ashworth scale score,Carroll hand function score and daily living ability score after treatment in the acupuncture group were lower than those before treatment(P<0.05),and the duration of spasm relief at the end of treatment was longer than that after the first treatment(P<0.05).The improved Ashworth scale score,Carroll hand function score and daily living ability score after treatment in the acupuncture group were lower than those before treatment(P<0.05).In the sham-acupuncture group and the control group,there was no significant difference in the improved Ashworth scale score,Carroll hand function score and daily living ability score after treatment compared with pre-treatment(P>0.05).After treatment,the effective rate of hand spasm relief,improvement of hand function and daily living ability in the acupuncture group was significantly better than those in the sham-acupuncture group and the control group(P<0.05),and there was no significant difference between the sham-acupuncture group and the control group(P>0.05).Conclusion: Acupuncture of "Feng fascia point" can effectively alleviate hand spasm immediately after stroke,and improve the hand function and daily living ability of patients,and the effect is more obvious with the prolongation of intervention time.
