Construction Of Nursing Clinical Program For Patients With Post Stroke Depression

Objective The purpose of this study is to analyze the current situation of clinical nursing practice of patients with post stroke depression,to search and evaluate the relevant guidelines,systematic evaluation or randomized controlled trials of post stroke depression,and to construct a nursing practice program for patients with post stroke depression based on guidelines and documentary evidence of traditional Chinese medicine,so as to provide a scientific reference for clinical nursing practice of patients with post stroke depression.Methods1.Through literature review,self-made "nurses' questionnaire for clinical nursing practice of post stroke depression" was formed after expert consultation and modification.A questionnaire survey was conducted among 58 neurological nurses in four third-class A hospitals and one community hospital in Shanghai by convenience sampling.2.Systematically searching domestic and foreign guideline websites and databases,screening and evaluating the quality of the guideline related to post stroke depression,content analysis of the included guideline,combing and integrating the recommendations related to nursing practice of post stroke depression patients.3.Systematically searching domestic and foreign databases,screening and evaluating the systematic evaluation or randomized controlled trials of the application of TCM appropriate nursing technology to patients with post stroke depression,analyzing the contents of the included literature,forming the text of recommendation related to TCM appropriate nursing technology,and judging the strength of recommendation related to TCM appropriate nursing technology through expert meeting method,and finally forming.Recommendation of appropriate medical care technology for patients with post stroke depression.4.Summarize the recommended opinions to form the first draft of the nursing practice program for post stroke depression patients,and further modify and improve it through expert meeting method to complete the construction of the nursing practice program for post stroke depression patients.By investigating the clinical applicability of nursing practice program for patients with post stroke depression,the possible resistance in clinical application of the program was analyzed,and the corresponding countermeasures were put forward.Results1.Through the questionnaire survey of 58 registered nurses in neurology department,the results showed that nurses had low awareness of observation,identification,evaluation,intervention and health education for patients with post stroke depression,and their implementation status was poor.There was no clear,scientific and unified nursing operation norm or nursing practice program for post stroke depression in clinic.Next step,we should fully consider post stroke depression in China.The current situation and existing problems of depression nursing practice were analyzed,and evidence-based evidence was integrated to construct a nursing practice program for patients with post stroke depression suitable for the current clinical environment in China.2.By systematically searching websites and electronic databases of the guidelines,204 guidelines were obtained,and finally 7 guidelines on post stroke depression nursing were included,and 39 recommendations were sorted out.3.By searching the Chinese and English databases,the researchers did not retrieve the relevant systematic evaluations that met the inclusion criteria.A further search of randomized controlled trials was conducted,and 15 randomized controlled trials on the application of appropriate nursing techniques of traditional Chinese medicine to patients with post stroke depression were included.Four recommendations were made.Through expert meeting method and drawing lessons from GRADE standard,the strength judgment of the four recommendation opinions is weak recommendation.4.The first draft of nursing practice program for patients with post stroke depression was formed by summing up the 43 recommended opinions.The final draft of nursing practice program for patients with post stroke depression was formed by further revising and perfecting with the opinions of 13 experts,which contained 31 recommended opinions.5.Through the clinical applicability evaluation of 15 neurological nurses,doctors,nursing managers and nursing educators,the results showed that the overall clinical significance and feasibility scores of the program were more than 80%.The respondents had different opinions on the clinical significance and feasibility of different modules of intervention,and the main influencing factors included the theoretical knowledge of nurses and the degree of cooperation of patients.Conclusions1.At present,there is no clear,scientific and unified nursing practice program for patients with post stroke depression.The evidence of Western medicine nursing and traditional Chinese medicine nursing should be integrated,and constructing evidencebased nursing practice program to guide clinical nurses' operation and improve the quality of nursing practice.2.The nursing practice program for post stroke depression patients is based on the current situation of nursing practice for post stroke depression in China,the best evidence,relevant evidence-based resources and the opinions of medical and nursing experts.
