Objective: In this study, patients with aphasia after acute ischemic stroke were taken as the research object. The acupuncture or non-acupuncture treatment was used to observe the recovery of aphasia at different time points of the patients, and the effect was evaluated. The necessity of training aims to explore the current trend of optimization of clinical treatment plans for patients with aphasia after acute ischemic stroke in China.Method: This study used a prospective observational study design and included 34 patients with aphasia after acute ischemic stroke in the acupuncture department of the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from January 2018 to December 2018.During the hospitalization, basic medical treatment and acupuncture were used to compare acupuncture. After 3 months of treatment and without acupuncture after discharge, the aphasia recovery degree of the two groups of patients was evaluated and the efficacy was evaluated. At the same time, the patients were followed up to 1 year after the onset of the disease. The Western Aphasia Test(WAB)and Boston Aphasia Severity Grading(BDAE),the National Institute of Neurological Impairment Score(NIHSS)language item score,to observe the recovery of language function of patients at different time points. The measurement data used paired sample t test,Friedman test and rank sum test;the grade data used Friedman test, rank sum test and Wilcoxon signed rank test;the result P<0.05 was regarded as significant difference and had statistical significance. The data obtained was processed using SPSS 25.0 statistical software.Result:(1)Baseline comparison: The rank sum test was performed on the indexes of the patients who were treated with acupuncture for 3 months and the patients who were not acupuncture after discharge from hospital, and there was no statistical difference(P>0.05),which was comparable.(2)Efficacy evaluation: BDAE scores and WAB scores of patients who did not receive acupuncture treatment after discharge from the hospital at 3 months after discharge and those who received acupuncture treatment for 3 months were statistically different(P>0.05)However,comparing the efficacy of acupuncture between these two groups of patients, there was no statistical difference(P>0.05).(3)Aphasia recovery during the follow-up period: The sub-items and total scores of the BDAE score,WAB score, and NIHSS language item scores of the included cases at different time points were statistically different(P<0.05).There was no statistically significant difference between NIHSS score language items at different time points(P>0.05).Among them, the BDAE score,WAB sub-items and total scores were significantly higher than those at admission and 1 month after onset 3 months,6 months after onset, and 1 year after onset(P<0.05);WAB repetition and total score After 6 months of onset and 1 year after onset,it was significantly higher than after 3 months of onset(P<0.05).Conclusion:(1)Patients with aphasia after acute ischemic stroke patients who did not undergo acupuncture after discharge and patients with acupuncture treatment for 3months had a recovery in language function 3 months after onset, but received acupuncture treatment 3 Compared with patients who did not receive acupuncture treatment after discharge from the hospital, patients at months did not show the therapeutic advantages of acupuncture.(2)Patients with aphasia after acute ischemic stroke have the best recovery during the period of 1-3 months, which is reflected in the amount of information, spontaneous language fluency, listening comprehension, and naming. After 3 months, there is still progress, and after 6 months, the overall language recovery will not change.(3)Psychological factors and family support have a great influence on the improvement of language function in patients with aphasia after stroke.
