Objectives 1.Investigate the general situation,social support,perception control,health literacy and self-management status of patients with coronary heart disease;2.Assess the correlation between social support,perception control,health literacy and self-management in patients with coronary heart disease;3.Analyze the influencing factors of self-management of patients with coronary heart disease;4.Use structural equation models to explore the relationship and degree of social support,perception control,and health literacy on self-management in patients with coronary heart disease.Methods In this study,the convenience sampling method was used to select 496 patients with coronary heart disease who were treated in two third-level Class A hospitals in Tianjin from June 2019 to October 2019.The self-designed general data questionnaire,social support assessment scale,revised version of control attitude scale,chronic disease patients' health literacy scale,coronary heart disease self-management behavior scale were used for investigation.Use SPSS 19.0 statistical software for data entry and descriptive analysis,single factor analysis of variance,t test,correlation analysis,multivariate stepwise analysis;use AMOS 23.0 for structural equation model analysis to establish social support,perception control,and health literacy in patients with coronary heart disease Relationship model with self-management.Results 1.The self-management score of patients with coronary heart disease is(82.22 ± 26.50),which is at a moderate level;the social support score is(26.57 ± 8.22),which is at a low level;the score of perceptual control is(25.25 ± 7.70),which is at a moderate level Level;The health literacy score is(73.67 ± 24.82),which is at a low level.2.The results of single factor analysis of variance for self-management of patients with coronary heart disease show that different genders,ages,marital status,education level,work status,occupation,monthly household income per capita,place of residence,method of payment for medical treatment,living conditions,type of disease,course of disease,There is a difference in the level of self-management among patients with coronary heart disease who have treatment methods and comorbidities.3.Correlation analysis results show that social support,perception control,and health literacy all have a positive correlation with self-management;social support interacts with perception control and has a positive correlation with health literacy;and perception control has a positive correlation with health literacy.4.Regression analysis results show that living with relatives,higher social support,perception control,health literacy level,married,retired or in employment,having comorbidities,and using urban medical insurance are factors that improve the self-management level of patients with coronary heart disease;Men,under secondary school education,acute coronary syndrome,monthly income <5000 yuan,farmers,individuals,workers are factors that reduce the self-management level of patients with coronary heart disease.5.The analysis results of the structural equation model show that health literacy plays a partial intermediary role in social support and self-management,perception control and self-management,and that sense control and health literacy plays a partial intermediary role in social support and self-management.Conclusion 1.The self-management of patients with coronary heart disease is at a medium level,the level of social support is at a low level,the level of perception control is medium,and the level of health literacy is low.2.Self-management of coronary heart disease patients with different genders,ages,marital status,education level,working status,occupation,family monthly income per capita,place of residence,payment methods for diagnosis and treatment,living conditions,disease type,course of disease,treatment methods,comorbidities There are differences in levels.3.Coronary heart disease patients' social support,perception control,and health literacy all have a positive correlation with self-management;social support interacts with perception control,and has a positive correlation with health literacy;perception control has a positive correlation with health literacy.The higher the level of patient social support and perception control,the higher the level of health literacy and the higher the level of self-management.4.Different living styles,social support,perception control,health literacy,marital status,working status,comorbidities,payment methods,gender,education,disease type,monthly household income per capita,and occupation affect the self-management level of patients with coronary heart disease the elements of.5.Social support can directly affect the self-management of patients with coronary heart disease,and can also indirectly affect self-management through the intermediary role of health literacy.Management;social support can also indirectly influence self-management through the intermediary role of perception control and health literacy.
