| Objective: To observe the therapeutic effect of transverse tibial bone transfer on thromboangiitis obliterans.Methods:(1)From January 2018 to August 2019,18 patients with thromboangiitis obliterans undergoing transverse tibial bone transfer were selected from the Affiliated Hospital of Youjiang University of nationalities.The changes of skin temperature,visual analogue score,blood oxygen saturation,ankle brachial index,VEGF-A concentration in serum,wound healing of ulcer,arteriography of lower extremity before and after operation were compared to observe the vascular regeneration of the affected limb after tibial transverse bone removal.(2)SPSS22.0software was used for statistical analysis.Results:(1)Skin temperature after surgery was gradually increased compared with that before surgery,and the skin temperature of the affected limb and toe was compared 1 day before surgery and 7,14 and 28 days after surgery(P<0.05),the difference was statistically significant.(2)The visual simulation score of the patients decreased gradually at 1 day before surgery and at 7,14 and 28 days after surgery(P<0.05),the difference was statistically significant.(3)The blood oxygen saturation of the toe of the affected limb increased gradually 1 day before surgery and 7,14 and28 days after surgery(P<0.05),the difference was statistically significant.(4)The ankle-brachial index of the affected limb increased gradually 1 day before surgery and7,14 and 28 days after surgery(P<0.05),and the difference was statisticallysignificant.(5)The serum vegf-a concentration of the patients was increased at 7,14 and 28 days after surgery(P<0.05),with statistically significant differences.(6)16patients with extremity ulcer and gangrene were basically healed after surgery,and 2patients were eventually amputated.(7)Arteriography of the affected limb before and after the removal of the stent showed that there were obvious small vessels and microcirculation in the soft tissue of the leg and foot of 15 patients compared with that before the operation,and they interwoven into a net to construct a new vascular network.No obvious angiogenesis was observed in 3 patients.Conclusion:Transverse tibial bone transfer can improve the symptoms of lower limb ischemia,microcirculation regeneration and ulcer wound healing in TAO patients.It has the advantages of simple operation and small trauma,and is worth clinical promotion. |
