Objectives:1.Through clinical randomized trials,the objective measures such as TCM symptom scale,SF-MPQ,SF-36 were used to accurately evaluate the clinical effect of acupuncture on migraine.Observing the effect of different needle retention time on acupuncture treatment of migraine,and providing a basis for the standardized treatment of acupuncture treatment of migraine for clinical application.2.Transcranial doppler ultrasonography(TCD)was performed before and after treatment to observe the effect of scalp acupuncture on cerebral blood flow.Methods:104 migraine patients were randomized into 3 groups,10-minute treatment group,20-minute treatment group,and 40-minute treatment group.They had to receive scalp acupuncture with different needel retention time,3 times a week,4 weeks for a course of treatment.The changes of TCM syndrome scale,SF-MPQ,SF-36 and TCD before and after treatment were observed.Results:1.The total effective rate of 10-minute treatment group,20-minute treatment group,40-minute treatment group were 64.5%,93.5%and 93.9%.The difference in total effective rate between the 20-minute treatment group and the 40-minute treatment group were statistically significant(P<0.05)after chi-square test.There was no statistical significance in total effective rate between the 40-minute treatment group and the 20-minute treatment group(P<0.05).2.SF-MPQ score:After nonparametric test,the difference of migraine scores before and after treatment in the three treatment groups was statistically significant(P<0.01).There was a significant difference between the 20-minute treatment group and the 40-minute treatment group compared with the 10-minute treatment group(P<0.01).There was no statistical difference between the 40-minute treatment group and the 20-minute treatment group(P>0.05).3.Migraine score:After nonparametric test,there was a statistical difference in migraine scores before and after treatment in groups(P<0.01).There was a statisticlly difference between the 20-minute group and the 40-minute group compared with the 10-minute treatment group(P<0.01).There was no significant difference between the 40-minute treatment group and the 20-minute treatment group(P>0.05).4.SF-36 score:After nonparametric test,there were statistical differences between the 20-minute treatment group and the 40-minute treatment group in improving the role-physical,social functioning,role-emotional,and mental health compared with the 10-minute treatment group(P<0.05),but there was no statistical difference between the two groups(P>0.05).There was no statistical difference in the improvement of body pain between the 20-minute treatment group and the 40-minute treatment group(P>0.05).There was no statistical difference in the improvement of body pain before and after treatment in 10-minute treatment group(P>0.05).There was no statistical difference in the improvement of physical functioning,general health and vitality between the three treatment groups(P>0.05).5.TCD:In patients with cerebral vasospasm,the peak systolic velocity of the basilar artery and the left and right vertebral artery in the three treatment groups were significantly decreased,and the difference was statistically significant(P<0.05),but there was no significant difference between the three groups(P>0.05).In patients with cerebral insufficiency,the peak systolic velocity of the basilar artery and the left and right vertebral artery in the three treatment groups increased significantly,and the difference was statistically significant(P<0.05),but there was no significant difference between the three groups(P>0.05).Conclusions1.The efficacy of the 20-minute treatment group and the 40-minute treatment group were better than that in the 10-minute treatment group There was no significant difference in efficacy between the 20-minute treatment group and the 40-minute treatment group.2.The improvement of migraine score,SF-MPQ score and some SF-36 items in the 20-minute treatment group and the 40-minute treatment group were better than those in the 10-minute treatment group,but there was no significant difference between 20-minute treatment group and 40-minute treatment group.3.Scalp acupuncture has a two-way regulation of cerebral blood flow in patients.There were no significant differences in the improvement of blood flow velocity between the three different needle retention times at 10 minutes,20minutes and 40 minutes.
