Purpose:On the basis of routine treatment and rehabilitation training, the clinical trial of acupuncture combined with low energy extracorporeal shock wave in the treatment of increased muscle tension in upper limbs after stroke was carried out and observed to evaluate the clinical effect of this scheme on the increase of muscle tension in upper limbs after stroke, and to evaluate its feasibility and safety,so as to optimize the treatment scheme and provide scientific basis for clinical application. Methods:From May 2019 to November 2019,61 patients who were treated in the outpatient department and inpatient department of the general rehabilitation department of the first affiliated hospital of Tianjin University of traditional Chinese Medicine were selected to meet the diagnostic and inclusion criteria required by the clinical trial.According to the sequence of visits, the patients were divided into 30 cases and 31 cases according to the random digital table method. The two groups received basic treatment and routine rehabilitation training for cerebrovascular disease, one week as a course of treatment,a total of 5 weeks of treatment, the two groups before treatment,1 week treatment.Three weeks after treatment, five weeks after treatment, using modified Ashworth index,FMA upper limb motor function scale, modified Barthel index rating scale(Modidified Barthel Index,MBI)for data collection and efficacy evaluation, and then using SPSS21.0 statistical analysis software to analyze the data, compare the difference of treatment effect between the two groups, and then draw a conclusion?Results:1.The total effective rate of the experimental group and the control group was 32.14%and 20.00% respectively after 1 week of treatment,57.14% and 37.67% respectively after 3weeks of treatment, and 85.71% and 53.33% respectively after 5 weeks of treatment,The results indicated that the efficacy of the experimental group was better than that of the control group.2.modified Ashworth index :(1)after 1 week treatment, there was no significantdifference between the modified Ashworth score of flexor muscle group and before treatment. After 3 weeks and 5 weeks treatment, the muscle tension score of flexor muscle group in upper limb was statistically significant compared with before treatment(P<0.05);(2)control group: after 1 week treatment compared with before treatment, the difference was not statistically significant;after 3 weeks treatment compared with before treatment, only flexor muscle group tension score was statistically significant(P<0.All muscle tone scores of flexor muscle group were uniform after 5 weeks treatment and before treatment.3.FMA upper limb motor function score :(1)after 1 week treatment compared with before treatment, the difference was not statistically significant,3 weeks and 5 weeks after treatment compared with before treatment, the difference was statistically significant(P<0.05);(2)control group: after 1 week treatment, the difference was not statistically significant,3 weeks and 5 weeks after treatment compared with before treatment, the difference was statistically significant(P<0.After 1 week of treatment, there was no significant difference between groups. After 3 weeks and 5 weeks of treatment, the difference was statistically significant(P<0.05).4.Modified Barthel index :(1)after 1 week treatment, the difference was not statistically significant after 3 weeks and 5 weeks treatment, compared with before treatment, the difference was statistically significant(P<0.05);(2)after 1 week treatment, the difference was not statistically significant after 3 weeks and 5 weeks treatment, compared with before treatment, the difference was statistically significant(P<0.(3)Inter-group comparison: After 3weeks and 5 weeks of treatment at 3 treatment nodes, the difference was statistically significant, indicating the efficacy of the experimental group in improving patients' daily living ability better than the control group.Conclusions:1.Acupuncture combined with low energy extracorporeal shock wave and acupuncture can effectively reduce the muscle tension level of each flexor group in the upper limb after stroke, and the effect of acupuncture combined with low energy extracorporeal shock wave is better than that of simple acupuncture.2.Acupuncture combined with low energy extracorporeal shock wave and acupuncture can improve the motor function of the affected upper limb, and the effect of acupuncturecombined with low energy extracorporeal shock wave is better than that of simple acupuncture.3.Acupuncture combined with low energy extracorporeal shock wave and acupuncture can improve the daily life ability of patients, and acupuncture combined with low energy extracorporeal shock wave is better than simple acupuncture.4.Acupuncture and low low-energy extracorporeal shock wave have no obvious adverse effects on the muscle tension of upper limbs after stroke.
