Objectives1 The purpose of this study is to understand the general situation,self-perceived burden,social support,fear of progression and the quality of life of patients with stroke,and to analyze the impact of different demographic data on the quality of life of patients with stroke;2 To analyze the correlation between self-perceived burden,social support,fear of progression and the quality of life in patients with stroke;3 To explore the role and intensity of self-perceived burden,social support,fear of progression and the quality of life in patients with stroke.MethodsThis study was a cross-sectional survey for the patients with stroke who were hospitalized in a third-grade hospital in Tianjin from July to November 2019 and met the exclusion criteria.A total of 583 patients were enrolled in the study.The questionnaires of general information,self-perceived burden scale(SPBS),social support rating scale(SSRS),fear of progression questionnaire-short form(Fo P-Q-SF)and stroke impact scale(SIS)were used as research tools for questionnaires.Descriptive analysis,t-test,one-way ANOVA,correlation analysis and multiple stepwise regression analysis were performed using the SPSS 21.0 statistical software package.AMOS 22.0 software was used to analysis structural equation model.Results1 The score of quality of life of patients with stroke was 445.14 ± 136.88;the score of selfperceived burden was 33.02 ± 6.08;the score of social support was 33.75 ± 6.63;the score of fear of progression was 33.76 ± 6.27.2 Comparing the scores of quality of life of patients with stroke with different demographic characteristics showed that there were statistically significant differences(P<0.05)in age,residence,marital status,living condition,caregiver,medical payment method,family per capita monthly income,education level,working condition,number of combined other chronic diseases and types of stroke,while there were no statistically significant differences(P>0.05)in gender,course of disease,family history of stroke and frequency of disease attack.3 The results of correlation analysis showed that the quality of life was negatively correlated with self-perceived burden and the fear of progression,and positively correlated with social support;self-perceived burden was negatively correlated with social support,and positively correlated with the fear of progression;Social support was negatively correlated with the fear of progression.4 The results of regression analysis showed that higher levels of social support and on-thejob were the factors to improve the quality of life;Fear of progression,self-perceived burden,?1000 yuan,1001?3000 yuan per capita household income and rural medical care ways were factors that lowered the quality of life.5 The results of structural equation model analysis showed that the data fit well with the theoretical model,and social support and fear of progression played an intermediary role between self-perceived burden and quality of life.Conclusion1 The self-perceived burden of patients with stroke is at a medium and low level,social support is at a medium and low level,the fear of progression is at a middle level,and the quality of life is at a medium level.2 There are differences in the quality of life of stroke patients with different age,residence,marital status,living conditions,caregivers,medical payment method,family per capita monthly income,education level,work status,number of combined other chronic diseases,types of stroke.3 the higher the level of self-perceived burden,the lower the level of social support,the higher the level of fear of progression,and the worse the quality of life of the patients.4 The quality of life of stroke patients can be directly affected by the self-perceived burden,social support and fear of progression.Self-perceived burden can also indirectly affect the quality of life through social support and fear of progression.
