| Purpose:To explore the correlation between yang-deficiency constitution and depression.To study the clinical effect of modified Fuzi decoction combined with Sertraline Hydrochloride in treating depression patients with yang-deficiency constitutionMethodsThis study is divided into two parts,from September 2017 to October 2019.Patients with yang-deficiency constitution depression,aged 18-65 years old,were collected from the outpatient department of psychosomatic diseases,affiliated hospital of Tianjin institute of traditional Chinese medicine.All patients in the group filled out Hamilton Depression Scale(HAMD-24,Hamilton Depression Scale-24)and Chinese Medicine Constitution Scale on the day of treatment(0 weeks).In the first part,the correlation between HAMD-24 scale score and TCM constitution scale score of the patients in the group for 0 weeks was analyzed.In the second part,the research method is clinical case-control study.The patients in the group are divided into control group and experimental group according to random number method.The control group should be treated with Sertraline Hydrochloride ranging from 100 to 200 mg/qd as appropriate.On the basis of the control group,the experimental group was additionally treated with Fuzi Decoction Jiawei Granules from Treatise on Febrile Diseases.Both groups of patients were scored with HAMD-24 scale and traditional Chinese medicine constitution scale at weeks 0,2,4,8 and 12.The therapeutic effect was evaluated according to the score reduction rate of HAMD-24 scale and the score reduction degree of traditional Chinese medicine constitution scale for patients at 0 and 12 weeks.The possible adverse reactions and residual symptoms occurred during the treatment of the two groups were observed.Statistical methods:The measurement data of normal distribution are described by(x±s)(mean standard deviation),while the skewness distribution is described by median M(QLP25,QUP75)(full range,quartile range);Pearson or Spearman correlation was used for correlation analysis.The difference analysis adopts repeated measurement variance analysis Chi-square test was used to compare the counting data.SPSS 17.0 was used for statistical analysis software,and the test level was ?=0.05.Bilateral tests were performed.P<0.05,the difference was statistically significantResults1 Correlation between yang-deficiency constitution and depression1.1 In patients with yang-deficiency constitution depression,yang-deficiency constitution score is moderately positively correlated with HAMD-24 score(r=0.508)(p<0.05);Yang-deficiency constitution score is positively correlated with low scores of anxiety/somatization(r=0.480,despair(r=0.364)and retardation(r=0.400))(p<0.05)Yang-deficiency constitution score is weakly positively correlated with cognitive impairment(r=0.284)and body weight(r=0.260)factor scores)(p<0.05),but is not correlated with day and night changes and sleep disturbance factors(P>0.05)1.2 There is a simple linear regression relationship between yang-deficiency constitution score and HAMD-24 scale score in yang-deficiency constitution depression patients(p<0.05)2 A Clinical Control Study on Fuzi Decoction Combined with Sertraline Hydrochloride in Treating Depression with yang-deficiency Constitution;2.1 Baseline ComparisonThere was no statistical difference in age,sex,HAMD-24 score and yang deficiency constitution score between the two groups(P>0.05),and the two groups were comparable2.2 Changes of HAMD-24 Scale Score2.2.1 The scores of HAMD-24 scale in the experimental group and the control group both decreased continuously(P<0.05)2.2.2 The scores of HAMD-24 scale in the 2nd,4th,8th and 12th weeks between the two groups were statistically significant(p<0.05).The score of the experimental group is lower than that of the control group2.3.3.The difference in effective rate between the two groups before and after treatment was statistically significant(p<0.05).The recovery rate of the experimental group was 38.55%and the total effective rate was 100%.The recovery rate of the control group was 4.82%and the total effective rate was 79.52%.The recovery rate and the total effective rate of the experimental group were higher than that of the control group2.3 Changes of Yang-Deficiency Constitution Score2.3.1 The yang-deficiency constitution score of the experimental group decreased continuously(p<0.05),while the yang-deficiency constitution score of the control group decreased continuously in 0-8 weeks(p<0.05),and remained basically unchanged in 8-12 weeks(P>0.05)2.3.2.The differences of yang-deficiency constitution scores between the two groups at the 2nd,4th,8th and 12th weeks were statistically significant(p<0.05).The score of the experimental group is lower than that of the control group2.3.3 The difference in effective rate between the two groups before and after treatment was statistically significant(p<0.05).The effective rate of the experimental group was 57.83%,and the total effective rate was 97.59%,while that in the control group was 0%and 69.88%.The effective rate and the total effective rate of the experimental group were higher than those of the control group2.4 Adverse Reactions Occurred in Patients with Yang-Deficiency Constitution Depression within 12 Weeks of TreatmentDuring the observation period,5 people in the experimental group had hand shaking,1 person had nausea and vomiting,4 people had daytime sleepiness,the incidence of adverse reactions in the experimental group was 12.05%;9 people in the control group had hand shaking,3 people had nausea and vomiting,11 people had daytime sleepiness,3 people had dizziness,2 people had abdominal diarrhea,the incidence of adverse reactions in the control group was 33.73%;the incidence of adverse reactions in the experimental group and the control group was 33.73%The difference was statistically significant(P<0.05).The incidence of adverse reactions in the experimental group was lower than that in the control group2.5 Occurrence of Residual Symptoms in Depression Patients with Yang Deficiency Constitution after 12 Weeks of Treatment2.5.1 Judging from HAMD-24 scale,after 12 weeks of treatment,the difference in anxiety/somatization factor,cognitive impairment and retardation factor scores between the experimental group and the control group is statistically significant(p<0.05).The difference in despair and sleep impairment factor scores between the experimental group and the control group is not statistically significant(p>0.05).After 12 weeks of treatment,the depression residual symptoms in anxiety/somatization factor,cognitive impairment factor and retardation factor of the patients in the experimental group are milder2.2.2 After 12 weeks of treatment,the difference in occurrence frequency of fatigue,constipation,pain and abdominal/hypochondriac distension in the experimental group and the control group was statistically significant(p<0.05).About to the residual symptoms of insomnia,the difference in frequency between the experimental group and the control group is not statistically significant(p>0.05).That is,after 12 weeks of treatment,the patients in the experimental group are less likely to suffer from fatigue,constipation,pain and abdominal/hypochondriac of depression residual symptomsConclusion:1 Yang-deficiency constitution is one of the risk factors of depression.In the depression patients with Yang-deficiency constitution,the score of yang-deficiency constitution can predict the total score of HAMD-242 Experimental group patients take Fuzi Decoction of modified combined with Sertraline Hydrochloride,and control group patients take Sertraline Hydrochloride.Two groups of treatment for yang-deficiency depression patients have curative effect,and the effective rate of the experimental group is higher than the control group,with fast onset,low incidence of adverse reactions and the residual symptoms were less and the degree was lighter 3 Fuzi decoction can improve the yang-deficiency constitution of depression patients. |
