Objective: To investigate the expression and clinical significance o f TP53 gene deletion in patients with chronic lymphoblastic leukemia(CLL).Methods: The clinical characteristics,immunophenotype,chromos ome karyotype,gene expression and FC regimen were analyzed retrospe ctively in 161 cases of CLL without P53 gene deletion and 32 cases of P53 genedeletion treated in our hospital from 2007 to 2017.The clinica l features,immunophenotypes,chromosome karyotype,gene expression a nd treatment of P53 gene deletion were analyzed.Results:1.The deletio n rate of P53 gene was 17%,and the stage of P53 gene deletion in CL L patients was higher than that in non P53 gene deletion group(P=0.03).2.The expression of P53 gene in CD38 of patients with P53 gene deletion was higher than that of patients without P53 gene deletion,and there was a significant difference between the two groups(P=0.01).In gene expression,there was a significant difference between P53 gene de letion CLL and non-P53 gene deletion CLL in(Immunoglobul invariabl e heavy chain region)Ig VH expression(P=0.04).3.In CLL patients tre ated with FC regimen,the total remission rate in CLL patients with P53 deletion was significantly lower than that in non-deletion patients.In this study,there was no significant difference in age,sex,blood cell co unt,immunophenotype and Ataxia telangiectasia mutated(ATM)gene d eletion between patients with P53 gene deletion and those without P53 gene deletion.Conclusion:1.CLL patients with P53 gene deletion tend to have higher stages,higher expression of CD38 and higher mutation r ate of Ig VH at the beginning of diagnosis,which provide reference for clinical intervention.2.The treatment effect of FC regimen in CLL pati ents with P53 deletion is poor,and the drugs affecting P53 signal trans duction system should be avoided.
