Epidemiology Characteristics Of MSM Population And Its HIV Infection In Guiyang From 2015 To 2019

Objective:To understand the characteristics of high-risk behavior and Human Immunodeficiency Virus?HIV?prevalence among men who have sex with men?MSM?in guiyang from 2015 to 2019,and provide evidence for prevention and control of HIV.Methods:The survey of HIV/AIDS sentinel surveillance was conducted among MSM during April to July from 2015 to 2019 in Guiyang,the data of the research were obtained from the information management system of AIDS thematic survey,this includes information on AIDS knowledge,drug use and sexual behaviour in the past six months and so on,the HIV prevalence and the change trend of HIV transmission influencing factors were analyze by Chi-square test and Cochran Armitege trend test.The factors for homosexual sex without condom and HIV infection were analyze by Logistic regression.Results:1,263 MSM were participated in the survey from 2015 to 2019,aged 18-63 years old?28.11±7.06?,and 86.8%?1096/1263?of the MSM were lived in the local area for more than 2 years,96.7%?1221/1263?were under 45 years old,unmarried people account for 87.3%?1102/1263?,the Han nationality account for 79.8%?1008/1263?,and 77.1%?974/1263?has college degree or above.The awareness rate of AIDS knowledge of the study subjects showed a decreasing trend year by year(c²=18.713,P=0.001;c²_trend=11.172,P=0.001),and the overall rate was 98.2%?1240/1263?.The multivariate analysis showed that peer education,HIV testing in the past year,and AIDS knowledge were protective factors for homosexual sex without condom.The proportion of peer education was 62.9%?795/1263?,overall increase(c²=101.936,P<0.001;c²_trend=38.035,P<0.001).In the last six months,the proportion of homosexual sex occurred was 92.2%?1165/1263?,which increased year by year in the last four years(c²=42.744,P<0.001;c²_trend=39.26,P<0.001).In the five years,the proportion of homosexual sex in the latest week occurred was 23.2%?57/246?,21.9%?46/210?,26.8%?59/220?,21.8%?50/229?,77.7%?202/260?,in total 1165 MSM,overall increase(c²=261.221,P<0.001;c²_trend=136.270,P<0.001),the proportion of homosexual sex commercial was increased in recent years(c²=49.996,P<0.001;c²_trend=37.65,P<0.001),the overall was 2.6%?33/1263?;During homosexual sex of MSM,6.3%?73/1165?never used condoms,38.2%?445/1165?of them sometimes used condoms,and 55.5%?647/1165?of them used condoms in each sexual.From2015 to 2019,there were 275 HIV positive cases detected,aged 18-59 years old?28.11±7.71?,The awareness rate of AIDS knowledge for HIV positive cases was98.5%?271/275?.The proportion of peer education was 61.1%?168/275?,overall increase(c²=35.158,P<0.001;c²_trend=25.251,P<0.001).In the last six months,the proportion of homosexual sex occurred for HIV positive cases was 92.7%?255/275?,which increased year by year in the last four years(c²=8.126,P=0.043;c²_trend=6.067,P=0.014),and 42.7%?109/255?of them used condoms in each sexual.The 5-year average detection rate of HIV infection was 21.8%?275/1263?,which showed a decreasing trend in the past three years(c²=7.148,P=0.028;c²_trend=6.492,P=0.011),and 18.2%?50/275?of Syphilis patients.Syphilis was detected in 7.8%?98/1263?.The prevalence of sexually transmitted diseases?STDs?in the last year was 3.8%?48/1263?.The rate of received Highly Active Antiretroviral therapy?HAART?for HIV patients was 73.1%?201/275?,and increased year by year(c²=20.109,P<0.001;c²_trend=15.960,P<0.001),97.0%?195/201?of them was suppressed.The multivariate analysis showed that nonlocal,has done the HIV test in the last year is the protection factor of HIV infection,homosexual anal sex without condom in the last six months,infection with syphilis is the risk factor of HIV infection.Conclusion:AIDS knowledge of MSM was not comprehensive and decreased in Guiyang.The proportion of receiving peer education was increased,while the proportion of other HIV/AIDS intervention programs receiving was decreased.Homosexual sex was active,heterosexual sex exists,and commercial sex was increased year by year.The MSM of seeking sexual partners from non-internet means has been increasing year by year.HIV infection rate,STDs infection rate and the complication of syphilis infection among MSM is high in Guiyang.HIV-infected people are more sexually active,anal homosexual sex without condom is common,and there is a risk of second-generation transmission.The anti-virus treatment rate and virus inhibition rate of HIV reached the standard.The cooperation between the disease prevention and control department and the MSM community group to carry out the HIV/AIDS has achieved obvious results.
