Expressions Of Serum Leptin And 25-Hydroxyvitamin D In Breast Cancer And Preliminary Discussion With The Efficacy Of Neoadjuvant Chemotherapy

Objective:The expression levels of serum leptin and25-hydroxyvitamin D(25(OH)D)in breast cancer patients were analyzed to explore the relationship between them and breast cancer.To preliminarily investigate whether neoadjuvant chemotherapy has an effect on the level changes of the two and whether the efficacy of neoadjuvant chemotherapy is related to the two.Methods:Sera and related clinical data were collected from 82 patients with primary breast cancer and 51 patients with benign breast disease confirmed by clinical histopathology.Enzyme-linked immunosorbent assay(ELISA)was used to detect serum leptin level,and electrochemical luminescence method was used to detect serum 25(OH)D level.Univariate analysis was used to compare the differences in age,body mass index(BMI),leptin,and 25(OH)D in breast benign disease group and breast cancer group.Factors with P<0.05 in univariate analysis were put into the binary classification in the logistic regression model,multi-factor analysis was performed by stepwise regression method to obtain the corresponding OR value and its 95%CI range.Statistical methods were used to analyze the relationship between serum leptin and 25(OH)D levels and different clinicopathological parameters.Thirty patients with breast cancer who underwent neoadjuvant chemotherapy were selected from the breast cancer group and evaluated for clinical efficacy after neoadjuvant chemotherapy.They were divided into the effective group(complete remission(CR)+partial remission(PR))and the ineffective group(stable disease(SD)+ Progressive disease(PD)).The serum of the last cycle of chemotherapy before neoadjuvant chemotherapy was collected.The detection methods of serum leptin and 25(OH)D are the same as above.Corresponding statistical methods were used to analyze thedifferences of serum leptin and 25(OH)D levels before and after neoadjuvant chemotherapy,and the differences of serum leptin and 25(OH)D levels before and after chemotherapy in the ineffective and effective groups.Results:1.Univariate analysis showed that:(1)The age of breast cancer group was significantly higher than that of benign breast disease group(P<0.05);(2)There was no significant difference between BMI in breast cancer group and benign breast disease group(P>0.05);(3)The level of serum leptin in the breast cancer group was significantly higher than that in the benign breast disease group(P<0.05);(4)There was no significant difference in serum 25(OH)D levels between breast cancer group and benign breast disease group(P>0.05).2.Multi-factor analysis:(1)Age is closely related to the occurrence of breast cancer(OR = 1.203,95% CI: 1.047 ? 1.382,P<0.05);(2)Leptin is closely related to the occurrence of breast cancer(OR = 2.226,95% CI: 1.227 ? 4.036,P<0.05).3.The clinical parameters of breast cancer patients such as BMI,age,menopausal status,TNM staging,breast cancer molecular typing,hormone receptor status,Her-2 gene and serum leptin and 25(OH)D levels were not significantly different(P All> 0.05).4.Analysis between changes in serum leptin and 25(OH)D levels and the efficacy of neoadjuvant chemotherapy:(1)The serum leptin level after neoadjuvant chemotherapy was significantly lower than before chemotherapy(P<0.05);(2)The serum leptin level in the effective group was significantly lower after chemotherapy than before chemotherapy(P<0.05);(3)There was no significant difference in serum leptin levels before and after chemotherapy in the ineffective group(P> 0.05);(4)There was no significant difference in serum 25(OH)D levels before and after chemotherapy,in the effective group and ineffective group before and after chemotherapy(all P>0.05).Conclusion:The results of this experiment show that leptin may be a risk factor for breast cancer,and leptin may play a role in the occurrence of breast cancer.Detection of serum leptin levels is expected to be a means of breast cancer screening;The serum leptin level decreased after neoadjuvant chemotherapy and decreased serum leptin levels in the effective group after chemotherapy suggest that it may be used as a serum indicator to judge the efficacy of neoadjuvant chemotherapy.The expression of serum 25(OH)D levels is not associated with breastcancer.The changes in levels before and after neoadjuvant chemotherapy have nothing to do with the efficacy of chemotherapy.It may be related to the experimental method or the small sample size in this experiment,which needs to be improved by experimental methods or expanded sample size and further research and verification.
