| In the first part.we summarized the traditional Chinese medicine cognition about the head necrosis(ONFH)in modern medicine.We also reviewed the progress of diagnosis and treatment about osteonecrosis of ONFH.In the second part:Objective:To study the therapeutic effect of Bushen Huoxue Decoction combined with varus osteotomy for the treatment of CJFHL1 type of ONFH.Methods:26 patients with 42 hips in the CJFHL stage 1 were randomly divided into 2 groups:11 patients(16 hips)in the single treatment group,while 15 patients(24 hips)in the combined treatment group.Both groups underwent varus osteotomy,and the combined treatment group took Bushen Huoxue Decoction 3 days after surgery for 180 days.We evaluated the effect in 3 months、6 months and 1 year after the operation.The imaging evaluation criteria developed by Professor Chen Weiheng were used to evaluate the femoral head after treatment with X-ray films.Morphological stability,and Harris hip function score scale was used to assess hip function.Results:The Harris scores of the two groups of patients before treatment had no significantly difference(t=0.413,P=0.874).After the opertion,the Harris scores of the two groups increased obviously.The Harris scores of the two groups of patients at 3 months postoperatively were 89.12±5.11 in the combination treatment group and 86.56 ±4.98 in the control group.The score of the group was higher than that of the control group(t=4.327,P=0.012).Bushen Huoxue Decoction combined treatment group stabilized 21 hips and aggravated 3 hips,and the control group stabilized 15 hips and aggravated 3 hips twelve months after treatment;there was no statistically significant difference in the stability of the morphological imaging of the femoral head between the two groups(X2=0.004,P=0.949).Conclusion:Bushen Huoxue Decoction combined with varus osteotomy can effectively prevent the femoral head from collapsing in patients with CJFHL1 and deficiency of liver and kidney in the short term after operation,and Bushen Huoxue Decoction can help to restore the hip function of patients in the early stage. |
