Effect Of Sitagliptin On Serum Irisin In Patients With Type2 Diabetes Mellitus?T₂DM? And Non-alcoholic Fatty Liver Disease?NAFLD?

Objective:To observe the effect of sitagliptin on blood glucose,liver enzymes,fatty liver and serum iris levels in patients with type 2 diabetes mellitus?T₂DM?combined with nonalcoholic fatty liver disease?NAFLD?.Method:Forty patients with T₂DM and NAFLD who were admitted to the First Affiliated Hospital of Xinjiang Medical University were randomly divided into observation group and control group,20 cases in each group.Both groups were given metformin?maximum dose:2000 mg/day?As a basic treatment,the observation group was treated with sitagliptin hydrochloride tablets?maximum dose:100mg/day?,and the control group was supplemented with repaglinide tablets?maximum dose:3.0 mg/day?.Fasting blood glucose,2-hour blood glucose,fasting insulin,glycosylated hemoglobin,insulin resistance index,blood lipids?triglycerides,cholesterol,high-density lipoprotein,low-density lipoprotein?,liver enzymes were recorded at baseline and after 12 weeks of treatment.?Alanine aminotransferase,aspartate aminotransferase?,serum iris?Irisin?and liver ultrasound.Results:?1?After treatment,the glycated hemoglobin,fasting blood glucose,2-hour blood glucose,fasting insulin,insulin resistance index,total cholesterol,and aspartate aminotransferase in the observation group decreased,and the difference was statistically significant?p<0.05?.Compared with the control group,the levels of glycated hemoglobin,fasting blood glucose,2-hour blood glucose,fasting insulin,and insulin resistance index decreased more significantly.?2?After treatment,the level of irisin in the observation group increased,the difference was statistically significant?p<0.05?.Conclusion:?1?Sitagliptin can not only improve glucose metabolism,lipid metabolism,but also improve liver function,and it is not significant for the improvement of fatty liver severity.?2?The efficacy of sitagliptin is better than that of repaglinide,which is expected to be the first-line treatment for T2DM with NAFLD.3sitagliptin is effective in the treatment of patients with type 2 diabetes mellitus complicated with nonalcoholic fatty liver disease,and its mechanism may be related to elevated serum Irisin levels.
