Objective:Analyze the risk factors of ovarian metastasis of endometrial cancer and explore the feasibility of ovarian preservation in young patients with early endometrial cancer.Methods:Retrospectively analyze the data of endometrial cancer patients who were initially treated in our hospital from 2016 to 2018.The 14 items such as age,tumor differentiation,lymph node metastasis,and fallopian tube infiltration processed by SPSS statistical software may be related to endometrial cancer metastasis to the ovary factor.Retrieve databases including pubmed,web of science,medline,embase,CNKI,etc.,screen documents and extract data according to inclusion and exclusion criteria,and perform meta-analysis of patients 'prognosis and quality of life through RevMan5.2.Results:The clinical stage,scope of the lesion,fallopian tube infiltration,lesion site,tissue type,ascites or peritoneal lavage fluid cytology,pelvic lymph node metastasis,vascular cancer thrombus,infiltrating muscle layer depth,degree of differentiation were statistically related to ovarian metastasis(P<0.05),Logistic multivariate analysis(from weak to strong): pathological stage <lesion site(corner angle)<differentiation degree <ascites cytology(positive)<pelvic lymph node metastasis <fallopian tube infiltration(P<0.05).Results of meta-analysis of ovarian-preserving young women:cancer recurrence rate[RR=1.33,95%CI(0.76,2.35),P=0.32],overall survival rate[RR=1.00,95%CI(0.99,1.01),P=0.88],prognostic analysis of deep muscular infiltration [RR=1.01,95%CI(0.87,1.17),P=0.93] were not statistically significant.The quality of life score [MD=13.20,95%CI(2.11,24.28),P=0.02] of the ovarian preservation group and the ovarian removal group was statistically different.The recurrence rate of the highly differentiated group was higher than that of the medium and low differentiation group [RR=0.35,95% CI(0.18,0.67),P=0.002] and therecurrence rate of the poorly differentiated group compared with the medium and high differentiation group[RR=0.25,95%CI(0.11,0.57),P=0.0009] The comparison is statistically different Conclusions:Young(?45 years old)and early(stage Ia)patients with well-differentiated endometrial adenocarcinoma consider that it is safe to retain the ovaries and improve the quality of life after surgery.
