Objective:To compare the dinical effect and safety of ultrasound-guided microwave ablation and traditional open surgery in the treatment of benign thyroid nodules.Methods:In this study,80 patients with benign thyroid nodules admitted to the first hospital ofh andan city for general surgical treatment from September 2017 to January 2019 were selected and divided into the microwave ablation group and the traditional surgery group according to their wishes,among which 34 patient chose microwave ablation treatment and 46 patients chose traditional open surgery treament.The rates of nodule resolution,average operation time,intraoperative blood loss,complications and average length of stay in the two groups were compared,and the changes in the rates of nodule resolution in the microwave ablation group at 1,3,6 and 12 months after surgery were analyzed.Results:1.In the microwave ablation group,the nodule regression rate was 15.6%,46.5%,67.8%and 82.6%at 1,3,6 and 12 months after operation,respectively.In the traditional operation group,the nodule regression rate was 100%.2.In the microwave ablation group,the bleeding volume was 13.35±3.57 ml,significantly less than 29.74±3.658 ml(t=-14.15,P=0.000)in the traditional operation group,and the average operation time was 33.53±3.02min,significantly shorter than 84.35±8.03 ml(P=-24.83,t=0.00)in the traditional operation group.There were significant differences in the amount of bleeding and the average operation time between the two groups(P<0.05).3.In the microwave ablation group,there were 3 patients with hematoma,1 patientwith skin burn,The incidence of complications was 11.8%;in the traditional operation group,there were 2 patients with hematoma,1 patient with infection,1 patient with groaning hoarseness,1 patient with parathyroid injury.1 patient with hypothyroidism,The incidence of complications was 13.0%.There was no significant difference in the incidence of complications between the two groups(P>0.05).There were no serious complications such as esophagus injury,trachea injury and death in both groups.In the microwave ablation group,1 case was converted to the traditional surgical treatment,because the broken tip of the microwave ablation knife could not be removed in the patient's body.During the microwave ablation,it was observed that the nodule was cystic and bulky.After ablation,it was converted to the traditional surgical treatment.During the operation,it was observed that the capsule wall of the thyroid nodule after ablation was not close to each other,and there was a posibility of recurrence.4.The average hospital stay of the whole group was(4.89±1.75)days the average hospital stay of the microwave ablation group was(2.94±0.93)days,the average hospstay of the tradtional operation group was(6.04±0.77)days,the average hospital stay of the microwave ablation group was significantly shorter than the average hospital stay of the operation group,the difference was statistically significant(t=-8.80,P=0.00).5.In the microwave ablation group,34 cases(60 nodules)were treated.The size of the nodules was observed by ultrasound at 1,3,6 and 12 months after the operation.The volume of the nodules gradually decreased with the incrrase of time.The volume of the nodules decreased greatly at 1-6 months and decreased at 6-12 months.In addition,there no recurrence or canceration in the microwave ablation group.Conclusions:Ultrasound-guided percutaneous microwave ablation of benign thyroid nodules is effective,with less complications,faster postoperative recovery.shorter hospital stay and better appearance.It is worthy of clinical application.
