Objective:To describe the compliance,psychological state and quality of life of patients with acute myocardial infarction;Analysis of the influencing factors on the compliance of cardiac rehabilitation exercise;To explore the effect of cognitive behavioral therapy on the compliance,psychological state and quality of life of patients with acute myocardial infarction,it provides theoretical basis for further improving patients' cardiac rehabilitation compliance in the future.MethodsThe first phase is an investigation study.In the first phase,220 patients with myocardial infarction were investigated from October 2018 to June 2019 in a thirdclass Hospital of Yangzhou.The survey tools used include general information questionnaire,heart rehabilitation exercise compliance questionnaire,Chinese cardiovascular patients' quality of life questionnaire,anxiety self-assessment scale.The second phase is an intervention study.In the second phase,according to the method of convenient sampling,120 patients with myocardial infarction were randomly assigned to the control group and the intervention group.The control group was given conventional cardiac rehabilitation guidance,and the intervention group was given short cognitive behavioral therapy intervention on the basis of the control group,twice a month,15 minutes each time,for 6 months.Short cognitive behavioral therapy is divided into two parts: cognitive therapy and behavioral therapy,two parts of the intervention were carried out at the same time.Cognitive therapy intervention includes four stages: psychological diagnosis,comprehension,cultivation and re education;Behavior therapy intervention includes three aspects: determining the appropriate treatment goal,selecting the appropriate treatment method and evaluating the treatment effect in time.Before the intervention,general information,compliance of cardiac rehabilitation exercise,quality of life,psychological state and related physiological indexes were evaluated.The patient's compliance of cardiac rehabilitation exercise was investigated again 3 monthons and 6 months after the intervention.Results1.The results of compliance of cardiac rehabilitation exercise in patients with acute myocardial infarction were poor in 135,general in 69 and good in 16;the overall score of anxiety was(60.78±7.24),23 with severe anxiety,93 with moderate anxiety and 90 with mild anxiety;the overall score of quality of life was(63.08±10.67).2.The differences in the compliance of cardiac rehabilitation exercise among patients with myocardial infarction in different demographic characteristics show that the compliance of cardiac rehabilitation exercise among patients with myocardial infarction has statistically significant significant differences in gender,age,treatment and other variables(P < 0.05).3.Before the intervention,the comparison results of general data,cardiac rehabilitation exercisecompliance,quality of life,psychological state and related physiological indicators of the two groups of patients showed that the differences were not statistically significant(P > 0.05).Comparison of the compliance between the two groups of patients with cardiac rehabilitation exercise: compared in the group,the compliance of the two groups of patients with cardiac rehabilitation exercise was higher than before(P < 0.05),however,after 3 months and 6 months of intervention,patients in the intervention group had better cardiac rehabilitation exercise compliance than those in the control group,and the difference was statistically significant(P < 0.05).The total score of quality of life in the two groups was significantly improved(P < 0.05),The difference between the two groups before and after the intervention was compared,and the improvement range of the intervention group was significantly higher than that of the control group(P < 0.05);Comparison of anxiety scores between the two groups: the improvement of anxiety in the intervention group was better than that in the control group after three months and six months of intervention(P < 0.05).The difference of anxiety scores between the two groups before and after intervention was higher than that in the control group(P < 0.05);Comparison of the blood pressure,LDL-C between the two groups of patients with cardiac rehabilitation,the blood pressure,LDL-C of the patients in the intervention group were lower than those in the control group(P < 0.05);But the body mass index and abdominal circumference the results showed that there was no significant difference(P > 0.05).Compared in the group,the blood pressure,LDL-C,body mass index and abdominal circumference between the intervention group and the control group were significantly lower than those before intervention(P < 0.05).Conclusions 1.The compliance of patients with acute myocardial infarction is not optimistic,so itis urgent to improve the compliance.2.The factors that affect the compliance of patients with acute myocardial infarction are gender,the number of patients with chronic disease,treatment and anxiety. Therefore,the short cognitive behavioral therapy intervention model can be used to improve the anxiety of patients,and develop personalized exercise methods for cardiac rehabilitation,so as to improve the compliance of patients with cardiac rehabilitation more effectively.3.Short cognitive behavioral therapy can improve the compliance of cardiac rehabilitation exercise and the rate of reaching the standard of physiological indexes in patients with myocardial infarction,and can improve the mental state and quality of life of patients with myocardial infarction.
