Objective:To explore the clinical value of Operator-CTO score in predicting the outcome of interventional therapy for chronic total occlusion of coronary artery. Background With the rapid development of technology and experience, the current CTO-PCI preoperative scoring model needs to be updated. Methods:The data of 144 CTO lesions in 130 CTO patients were analyzed prospectively. The operation was performed by 10 operators with different techniques and experiences. Before the operation,J-CTO,PROGRESS,ORA,Recharge and Operator-CTO scores were performed respectively. The clinical and operative data of different Operator-CTO score groups and different operators were compared. Finally, the predictive ability of each score to the outcome of CTO-PCI operation was observed and compared. Results:The total technique and success rate of operation were 90.9% and 88.9%,respectively.According to the "simple(? 2 points),medium(3 points),difficult(4 points)and extremely difficult(? 5 points)" graded by Operator-CTO,the technique success rate of CTO-PCI showed a decreasing trend(99%,87.5%,53.8% and 25%,respectively).The five scoring models had good calibration ability, and the curve below the Operator-CTO scoring curve showed a good calibration ability The product of 0.901(95% CI: 0.821-0.982,P < 0.01)was larger than the area under the curve of the other four scoring models, showing excellent ability to predict the technical outcome. Conclusion:the Operator-CTO score is a new clinical scoring tool that can predict the outcome of CTO-PCI,and can be used to grade the difficulty of surgery, which may play a good role in the majority of operators.
