Objectives:To elucidate the cognitive function characteristics of patients with comorbid metabolic syndrome and depression and its correlation with the diagnostic indicators of metabolic syndrome(triglycerides,high-density lipoprotein cholesterol,systolic blood pressure,diastolic blood pressure,fasting blood glucose,blood glucose after 2hours of glucose load,body mass index),anxiety and depression levels.Methods:1.By the method of clinical epidemiology,randomly selected from June 2018 to December 2019 in 3 armour hospital affiliated to a university during the first option of the psychosomatic medicine clinic patients as the research object,according to the standard set,elimination measures after the final into group(MDD),39 cases of patients with depression,depression comorbidity in patients with metabolic syndrome(MDD group + MS)38 cases.Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)were used to evaluate the severity of anxiety and depression.Montreal cognitive assessment scale(MoCA),logical memory test(LM),tower of London experiment(TOL)and number erasure test(NCT)were used to evaluate the cognitive impairment.Fasting blood glucose(FBG),blood glucose(2hPG),triglyceride(TG)and high-density lipoprotein(HDL)after 2 hours of glucose load,and diastolic blood pressure(DBP),systolic blood pressure(SBP),and body mass index(BMI)were measured.2.Describe and compare the differences in anxiety,depression and cognitive function between the MDD group and the MDD+MS group.On this basis,the correlation between the cognitive function of MDD+MS group and the diagnostic indicators of metabolic syndrome and the levels of anxiety and depression was analyzed.Research results:1.The results of cognitive function data showed that the total MoCA score,visual space,executive function and delayed memory scores of the MDD+MS group were lower than those of the MDD group,with statistically significant differences(P< 0.05).The TOL completion time and error number in MDD+MS group were higher than that in MDD group,and the correct number was lower than that in MDD group,with statistically significant differences(P < 0.05).LM scores of immediate memory and delayed memory in MDD+MS group were lower than those in MDD group,and the difference was statistically significant(P < 0.05).The NCT net score of MDD+MS group was lower than that of MDD group,and the error rate was higher than that of MDD group,the difference was statistically significant(P < 0.05).2.HAMD and HAMA data results showed that the HAMD total score,cognitive factor,somatization factor and despair factor scores of the MDD+MS group were higher than those of the MDD group,with statistically significant differences(P <0.05).There was no statistically significant difference in the scores of block factor,day and night change factor,sleep factor and body weight factor between the two groups(P > 0.05).HAMA total scores and scores of mental and physical anxiety factors in MDD+MS group were higher than those in MDD group,with statistically significant differences(P < 0.05).3.The total MoCA scores of the MDD+MS group were moderately negatively correlated with the total TG,FBG and HAMD scores,with significant differences(r=-0.53,r=-0.52,r=-0.66,P < 0.05).TOL completion time was moderately positively correlated with the total HAMD score,FBG and TG,with significant differences(r=0.57,r=0.45,r=0.49,P < 0.05).LM immediate memory scores were moderately negatively correlated with FBG and HAMD scores,with significant differences(r=-0.52,r=-0.46,P < 0.05).The net NCT score was moderately negatively correlated with HAMD,FBG and TG,with significant differences(r=-0.48,r=-0.53,r=-0.49,P< 0.05).Research conclusions:1.Both patients with MDD comorbidities and those with MDD alone havecognitive impairment,but MDD comorbidities have more serious impairment in visual space and executive function,delayed memory,immediate memory,and attention.2.The degree of anxiety and depression of MDD comorbid MS patients is more serious than that of MDD patients alone.3.The degree of cognitive impairment in MDD comorbid MS patients may be positively correlated with the severity of depression,triglycerides and fasting blood glucose levels.
