Objectives:To explore and analyze the application effect of case management model in patients with high-risk diabetic foot.Provide a theoretical basis for improving the health management of patients with high-risk diabetes.Methods:90 patients with high-risk feet who were treated in the diabetic foot disease clinic of the Endocrinology Department of a tertiary hospital in Jiangxi Province from June 2019 to July 2019 were selected as the research objects.The patients were divided into experimental group and control group according to the time of patient visit.45 patients who visited the hospital from June 1 to June 30 were the control group and 45 patients who visited the hospital from July 1 to July 31 were the experimental group.The patients in the control group were given routine nursing intervention in the diabetic foot disease clinic,and the patients in the experimental group were given case-based nursing intervention.Set up a case management research group;establish a case management process for high-risk diabetes patients;implement a case management model process.The changes of foot screening indexes,body mass index,CNAFF score and laboratory indexes?FBG?P2BG?HbA1C?blood fat?of the two groups of patients before and after intervention were compared.Results:1.Compared with general data such as gender,age,BMI,comorbidities,and foot condition,the difference between the two groups was not statistically significant?P>0.05?,and was clinically comparable.2.The difference in the incidence of diabetic foot ulcer between the two groups before and after intervention was statistically significant?P<0.05?.3.After intervention,the foot screening indexes ABI,VPT and TcPO2 in the experimental group were significantly improved compared with the control group,and the difference between the two groups was statistically significant?P<0.05?.4.After intervention,the body mass index of the experimental group was significantly lower than that of the control group,the difference was statistically significant?P<0.05?.5.After intervention,the CNAFF knowledge score of the experimental group was higher than that of the control group,the difference was statistically significant?P<0.05?.6.After the intervention,the blood glucose,2h postprandial blood glucose and glycated hemoglobin of the experimental group were lower than the control group,the difference was statistically significant?P<0.05?.7.After intervention,the blood fat the experimental group were better than those in the control group,and the difference was statistically significant?P<0.05?.Conclusions:1.Case management can regulate the blood lipid and blood glucose levels of diabetic patients and reduce the body mass index of patients;2.Case management mode can improve the foot function index,improve the foot care behavior and reduce the risk of diabetic foot.
