ObjectiveTo explore the effect of endocystectomy and part subadventital cystectomy in the treatment of hepatic echinococcosis adjacent to the hepatic porta.Methods1.From November 2005 to January 2020,we collected the cases of hepatic cystic echinococcosis adjacent to the hepatic porta treated by part subadventital cystectomy and endocystectomy.2.To sort out the general data of the collected cases(gender,age,contact history in pastoral area);3.To sort out the clinical data of the collected cases: the clinical manifestations(including abdominal distention,abdominal pain,fever,shivering,shock,etc.)on admission;4.To collect the data during the operation: the operation methods(part subadventital cystectomy and endocystectomy)were performed.The size and number of Echinococcus granulosus cysts and their relationship with hepatic porta.The average postoperative hospitalization days,incidence of postoperative residual cavity complications(infection,effusion,bile fistula),catheterization time,operating time,recurrence and death rate were analyzed retrospectively.ResultsAmong them,17 patients(47.22%)received endocystectomy as group endocystectomy,24patients(52.78%)received part subadventital cystectomy as group part subadventital cystectomy.36 cases were cured and 0 died.The number of days with catheter and the incidence of complications in the residual cavity in the group endocystectomy were significantly higher than that in the group part subadventital cystectomy,and the difference of the average postoperative hospitalization days between the two groups was not significant.All patients were followed up for 1-15 years,and there was no recurrence at present.ConclusionThe incidence of postoperative complications such as infection of residual cavity and biliary fistula in part subadventital cystectomy is relatively low,and the damage to liver is minimized.For hepatic echinococcosis adjacent to the hepatic porta,it can be preferred.
