Object:By applying the information early warning and monitoring scheme based on the adjusted MEWS score to the patients after general anesthesia in general surgery,the clinical application effect of the information early warning and monitoring scheme was observed,and its applicability in clinical was discussed,so as to provide a simple,rapid and real-time effective management tool for the clinical nurses to dynamically and scientifically evaluate the patients with "potential critical illness" after operation.Methods:According to the method of clinical experimental study,the patients who were hospitalized in the general surgery ward of a second class a hospital in Pudong New Area of Shanghai from January 2019 to June 2019 were divided into two groups: the experimental group(60 cases)and the control group(60 cases).The patients in the control group were monitored according to the doctor's orders and grading nursing standards after general anesthesia surgery Group B adopted the information early warning and monitoring scheme based on the adjusted mews.At the time of discharge,general data and disease-related data of patients,satisfaction of doctors,nurses and patients were collected.By using the method of quantitative study,the number of monitoring vital signs,the incidence of nursing adverse events,the satisfaction of nurses,the length of stay after operation,the rate of transfer to ICU,the standard rate of nursing records,the satisfaction of doctors and patients were compared between the two groups to evaluate the clinical application effect of the information early warning and monitoring scheme;by using the method of qualitative research,the experiment was conducted Group B participated in a semi-structured interview with nurses to evaluate the acceptance,satisfaction and suggestions of the information early warning and monitoring program.SPSS 22.0 statistical software was used for data analysis.The gender,age,education level,payment method of medical expenses,disease type,operation site,chronic disease,operation type,operation classification,operation method and postoperative complications of the two groups were described by frequency,composition ratio,mean and standard deviation,and statistical inference was made by two independent samples t test,rank sum test and chi square test.After the intervention,the time of hospitalization and the times of monitoring vital signs of the two groups were compared.The data obeyed the normal distribution,and the mean and standard deviation were used for statistical description.The t-test of two independent samples was used for statistical inference.If the normal distribution was not obeyed,the median and quartile spacing were used for statistical description,and the rank sum test was used for statistical inference.The nursing of the two groups was not the incidence of good events,the rate of transfer to ICU,the standardized rate of nursing records and the satisfaction of doctors,nurses and patients were described by frequency and composition ratio,and statistical inference was made by four grid chi square test.All test levels ? = 0.05,P < 0.05 was statistically significant.Results:1.There were 120 subjects in this study,60 in the experimental group and 60 in the control group.There was no significant difference between the two groups in gender,age,education level,payment method of medical expenses,disease type,operation site,chronic disease,operation type,operation classification,operation method and postoperative complications(P > 0.05).2.After intervention,the number of vital signs monitoring in the experimental group was 47.5(41)times,lower than that in the control group 64(49.25),and there was no significant difference between the two groups(P > 0.05).3.After intervention,the incidence of nursing adverse events in the experimental group(3.3%)was lower than that in the control group(13.3%),and the satisfaction of nurses in the experimental group(88.2%)was higher than that in the control group(35.7%).The difference between the two groups was statistically significant(P < 0.05).4.After the intervention,the postoperative hospital stay of the patients in the experimental group was(11.02 ± 4.090)days,less than that in the control group(13.85 ± 7.274).The difference between the two groups was statistically significant(P < 0.05).5.After intervention,the rate of patients transferred to ICU in the experimental group(1.7%)was lower than that in the control group(3.3%),there was no significant difference(P < 0.05).6.After intervention,the standard rate of nursing records in the experimental group(96.7%)was higher than that in the control group(85%);the satisfaction rate of doctors in the experimental group(93.3%)was higher than that in the control group(46.2%);the satisfaction rate of patients in the experimental group(96.7%)was higher than that in the control group(83.3%);the differences were statistically significant(P < 0.05).7.The results of qualitative interview showed that the nurses in the experimental group had a better acceptance and satisfaction of the information-based early warning and monitoring scheme based on the corrected MEWS.Conclusion:1.The information-based early warning and monitoring scheme based on corrected MEWS can help nurses identify and dynamically monitor the patient's condition changes,prevent the occurrence of nursing adverse events,and do not increase the workload of nurses in the process of condition monitoring.2.The information-based early warning and monitoring scheme based on corrected MEWS can promote the effective communication between doctors and nurses,improve the standardized rate of nursing records,the satisfaction of nurses and the effective utilization rate of medical resources.
