| Background: Ankylosing spondylitis(AS)is a kind of autoimmune disease.The etiology and pathogenesis are still unclear.Current studies suggest that genes,immune system and environmental factors all play a role in AS.Human leukocyte antigen B27(HLA-B27)has been confirmed to be directly related to the pathogenesis of AS.Tumor necrosis factor α(TNF-α)has become the target of treatment,intestinal flora disorders gradually attract people’s attention,and cytokines,especially IL-23/IL-17 axis,are the focus of current research.Periostin(POSTN),an extracellular matrix protein,is abnormally expressed in AS,but its exact mechanism needs to be further clarified.Interleukin-13(IL-13)is a pleiotropic cytokine,and it is still unclear whether it’s involved in the pathogenesis of AS.Interleukin-6(IL-6)is an important pro-inflammatory mediator that mediates inflammation in AS.The role of interleukin-1β(IL-1β)and interleukin-8(IL-8)in AS is unclear.Objectives: The serum levels of POSTN,IL-13,IL-6,IL-1β,and IL-8 in the patients with AS were detected,in the meantime,their correlations with clinical indicators were analyzed to explore their roles in AS.This study aims to further understand the pathogenesis of AS and provide a new target for the treatment of AS.Methods:A total of 34 patients with AS were recruited from the Department of Rheumatology and Immunology in China-Japan Union Hospital of Jilin University,all of them conform to the AS New York standard revised in 1984.Meanwhile,12 healthy subjects of the same period were selected as the control group.All of AS patients were treated with meloxicam and recombinant humanⅡtype of tumor necrosis factor receptor-antibody fusion protein.We collected clinical data from AS patients,including gender,age,immunoglobulin,complement,C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),bone mineral density(BMD),Ankylosing Spondylitis Disease Activity Score-C-Reactive Protein(ASDAS-CRP)and Bath Ankylosing Spondylitis Disease Activity Index(BASDAI),Bath Ankylosing Spondylitis Functional Index(BASFI),Bath Ankylosing Spondylitis Measure Index(BASMI),etc.The serum levels of POSTN,IL-13,IL-6,IL-1β and IL-8 were detected by enzyme-linked immunosorbent assay(ELISA),and their correlations with clinical indicators were analyzed.Results:(1)The serum levels of POSTN in the AS group were significantly lower than control group,and after treatment,the levels were higher than baseline with a statistically significant difference(P<0.05).(2)The serum levels of IL-13 in the AS group had an increasing trend compared with control group,which was not statistically significant(P>0.05).After treatment,the serum levels of IL-13 were higher and it’s higher than that of the control group(P<0.05).(3)The serum levels of IL-6 were positively correlated with ASDAS-CRP(r=0.750,P=0.000),BASDAI(r=0.760,P=0.000),BASFI(r=0.529,P=0.001),BASMI(r=0.513,P=0.003),ESR(r=0.558,P=0.001)and CRP(r=0.696,P=0.000),Ig G(r=0.374,P=0.030),Ig A(r=0.357,P=0.038),C3(r=0.381,P=0.026),and C4(r= 0.390,P=0.023).(4)The serum levels of IL-13 were positively correlated with lumbar vertebral bone mineral density(r=0.382,P=0.028).Conclusion: 1.The serum levels of POSTN in AS patients were significantly lower than control group,and after treatment,the levels were higher than baseline,which suggested that POSTN may have a protective effect on patients with AS.2.After treatment,the serum levels of IL-13 were higher than control group.The serum levels of IL-13 in AS were positively correlated with BMD.These data suggested that IL-13 may play an important role in controlling inflammation,reducing disease activity and bone protection.3.The serum levels of IL-6 were positively correlated with inflammatory indicators,disease activity and functional scores of AS,which suggested that IL-6 receptor antagonist may be an effective treatment for AS. |
