| Background:Ovarian endometriosis(OEM)is a kind of common benign gynecological diseases with malignant biological behavior.Endometriosis associated ovarian cancer(EAOC)is a special type of ovarian cancer.With the further understanding of "dual pathogenesis model" of ovarian cancer,some scholars think that atypical endometriosis(aEM)is a kind of precursor disease of ovarian cancer related to endometriosis.In molecular genetics,pathology and epidemiology,OEM has been proved to be a risk factor for ovarian cancer.In recent years,there are more and more reports about endometriosis related ovarian cancer at home and abroad.Both clinical and basic studies have confirmed that it is a specific type of ovarian cancer,and the research on its etiology,pathogenesis and clinical characteristics has become a hot spot.Objective:1.To analyze the general clinical characteristics of endometriosis related ovarian cancer for early diagnosis and treatment.2.To explore the risk factors of endometriosis related ovarian cancer.Methods:1.41 patients(EAOC group)with endometriosis related ovarian cancer diagnosed pathologically in the second hospital of Jilin University from January 2013 to September 2019 were collected.The medical records of each patient were reviewed in detail,and the clinical characteristics of each group were analyzed retrospectively and compared.2.Randomly select 70 cases(OEM group)of ovarian endometrioid cyst diagnosed by pathology from January 2013 to September 2019 in the second hospital of Jilin University,and check the medical records of each patient in detail.And 41 cases of endometriosis related ovarian cancer(EAOC group)were compared.The clinical characteristics of the two groups were compared,and the significance of each index in the diagnosis of endometriosis related ovarian cancer was calculated by calculating the area under ROC curve.Results:1.The average age of onset of endometriosis related ovarian cancer was 50.15 ± 9.073 years,the average age of menarche was 14.21 ± 0.26 years,the average number of pregnancies was 2.21 ± 0.21,the average number of births was 1.00 ± 0.09,36.6% of the patients had CA125 > 200 U/ml,the average level of CA199 was 192.77 ± 72.34u/ml,the average level of HE4 was 157.85 ± 182.78pmol/l,and the average diameter of tumor was 10.69 ± 0.57 cm.In the history of endometriosis related ovarian cancer patients,the main complaints were "pelvic mass found in physical examination" and "abdominal pain",with the proportion of 26.8% and 24.4%,respectively.Ultrasound showed that the mass was "mixed echo of cyst and solid",with the highest proportion of 53.7%,dysmenorrhea rate of 46.3%,menstruation regularity of 65.9%,the main lesion involved was unilateral ovary,FIGO stage mainly in early stage(97.6%).Compared with OEM group,EAOC group had statistical significance in age of onset,menstrual regularity,dysmenorrhea,pregnancy times,complaints,serum CA125 value,ultrasound indicating the nature of pelvic mass and tumor diameter(P < 0.05).2.Using logistic single factor analysis,age,dysmenorrhea,menstrual regularity,pregnancy times,complaints,serum CA125 value,ultrasound suggested that the nature of pelvic mass and tumor diameter were independent risk factors of EAOC,while menarche age,birth times,serum CA199 value,tumor side and gynecological disease history were not independent risk factors of EAOC.Age and tumor diameter were independent risk factors for predicting endometriosis related ovarian cancer.Conclusions:1.The age of onset of endometriosis related ovarian cancer patients is mostly at the level of perimenopause.The main complaints are pelvic mass and abdominal pain,and the tumor diameter is more than 10 cm,the CA125 level is significantly increased,and the clinical stage is mostly early.The patients with the above clinical characteristics suggest that the possibility of endometriosis related ovarian cancer is high.2.According to logistic single factor and multi factor analysis,age and tumor diameter were independent risk factors for predicting endometriosis related ovarian cancer. |
