Background:Thyroid carcinoma is the most common endocrine malignancy.In the past30 years,the incidence rate of thyroid carcinoma is increasing all over the world.Papillary thyroid carcinoma is the most common type of pathology of thyroid cancer.The detection rate of micropapillary thyroid carcinoma increased with the improve of thyroid cancer diagnostic techniques.But the mortality rate of thyroid carcinoma has remained fairly stable.Some researchers have suggested that micropapillary thyroid carcinoma have been overdiagnosed and overtreated,they proposed that active surveillance can be the first-line management for low-risk micropapillary thyroid carcinoma.Subclinical hypothyroidism is common thyroid diseases.The incidence is higher in female than male.The incidence of thyroid cancer patients with subclinical hypothyroidism is 10.7%.Objective:Our study assesses the effect of preoperative subclinical hypothyroidism on the tumour's clinicopathological features of micropapillary thyroid carcinoma.And explore the role of subclinical hypothyroidism in the evolution of micropapillary thyroid carcinoma: Push? protection? Meaningless? Based on the results of the above discussion to guide cinical treatment of the micropapillary thyroid carcinoma patients with subclinical hypothyroidism who choose close surveillance instead of surgical treatment.Method:1.7410 patients who underwent initial surgical treatment for micropapillary thyroid carcinoma in the First Hospital of Jilin Universitybetween January 2009 and December 2019 were enrolled in this study.2.Collate the collected case information,including the patients' basic condition,laboratory examination results,postoperative paraffin pathological results.3.Preoperative micropapillary thyroid carcinoma patients were classified according to whether they had subclinical hypothyroidism and the severity of subclinical hypothyroidism.4.Retrospective analysis of the effect of preoperative subclinical hypothyroidism and different level of subclinical hypothyroidism on the tumor's clinicopathological features of patients with micropapillary thyroid carcinoma.5.SPSS for Windows(version 23.0;SPSS)was used to analyse the data.The inspection standard is ?=0.05,statistical significance was defined as p < 0.05.Result:1.The incidence of micropapillary thyroid carcinoma with subclinicalhypothyroidism is higher in female than male.2.Micropapillary thyroid carcinoma patients with subclinicalhypothyroidism has a larger tumor size than micropapillary thyroidcarcinoma and is more prone to extra-thyroidal extension,lateral neckdissection lymph node metastasis and chronic Lymphocytic Thyroiditis.3.There was no significant difference between the tumor's clinicopathological features of patients with severe subclinical hypothyroidism and mild subclinical hypothyroidism.Such as,primary tumor size,extra-thyroidal extension,bilateral carcinoma,multifocality and lymph node metastasis.4.The incidence of chronic Lymphocytic Thyroiditis of micropapillary thyroid carcinoma patients with Severe subclinical hypothyroidism is higher than micropapillary thyroid carcinoma with mild subclinical hypothyroidism.Conclusion:Subclinical hypothyroidism has some influence on the disease progression of micropapillary thyroid carcinoma.In the process of diagnosis and treatment of micropapillary thyroid carcinoma,the impact of subclinical hypothyroidism should be fully considered.
