Analysis Of Drug Use In A Community Health Service Center In Shanghai In 2018

With the continuous progress of medical reform in recent years,the cause of medical and health care has evolved by leaps and bounds,and a lot of great progress has been made.As an important part of grass-roots medical and health institutions and community construction,more and more people has paid attention to community health service center and recognized its importance.Community health service is the basic medical and health service for the broad masses of the people,and it is an important guarantee to improve the health level of the people.The development of community health service is of great significance to facilitate the masses to seek medical treatment,to solve the problems of "difficulties in seeing a doctor" and "expensive medical treatment",to save health resources and to strengthen the construction of socialist spiritual civilization.The counterpart community of our center is Xinqiao Town with 194000 people,Xinqiao Town is located in the southwest suburbs of Shanghai city,northeast of Songjiang district,the medical resources of this area are relatively not concentrated,there is only one level I medical and health unit in Xinqiao Town.As the only level I medical institution in our town,it plays an important role in protecting the people's health,and it's very important to develop the center.Based on the statistics and analysis of the overall situation of sampling prescriptions,drug sales and the use of four kinds of drugs(antihypertensive drugs,hypoglycemic drugs,anti-stasis drugs,respiratory drugs)in our center from January to December of 2018,this paper provides a reference for the later community optimization reform and help for the management of drug purchase and supply reserve,plays a supporting role in the implementation of the relevant national system,and provides a reference for clinical right drug use.This paper is divided into six chapters,the main contents include:The background and significance of the research subject are introduced in the first chapter,and the significance of the development and the analysis of drug use of community center is expounded.The basic situation of our center,the overall situation of prescription and the overall sale of drugs in our center are analyzed and the reasons that four kinds of drugs are selected to be further analyzed is stated.The use of antihypertensive drugs is analyzed in the second chapter,and the situation of hypertension contracted patients in 2018 is analyzed from the aspects of sex,age,complications,combined use of drugs and so on.The varieties,drug specifications and sales amount of all antihypertensive drugs from January to December of 2018 are classified and counted by Excel software.The limited daily cost(DDC)analysis method and the amount ranking method,the five kinds of antihypertensive drugs commonly used in our center were analyzed by using the limited daily dose(DDD)analysis method recommended by WHO,and the overall situation and future development trend of antihypertensive use in our center are summarized according to their respective action mechanisms and clinical application characteristics.The use of hypoglycemic drugs,including insulin injection and oral hypoglycemic drugs is analyzed in the third chapter.The general situation of diabetes mellitus in China is introduced from two aspects: the prevalence rate of diabetes mellitus and the cost of direct medical treatment in this paper,and the situation of the contracted patients of diabetes mellitus in 2018 is analyzed.from three aspects: age,sex and drug use.The variety,drug specification,usage quantity and sales amount of all hypoglycemic drugs in 2018 are classified and compared by using Excel software.Using the limited daily dose(DDD)analysis method recommended by WHO,the limited daily cost(DDC)analysis method and the amount ranking method,combined with their respective characteristics,diabetes prevention and treatment guidelines recommended drugs,7 kinds of insulin preparations and 17 kinds of oral hypoglycemic drugs in our center are comprehensively analyzed.We understand the shortcomings and the future learning direction by studying one case of the use of hypoglycemic drugs of diabetes mellitus combined with hypertension in our center.The use of antistasis agents is analyzed in the fourth chapter.Excel software is used to classify and summarize the varieties,quantity and sales amount of anti-stasis agents in 2018.According to the drug instructions,the functional treatments of 32 antistasis agents are analyzed and summarized,and 28 drugs are obtained for the treatment of cardiovascular and cerebrovascular diseases,mainly for the treatment of chest arthralgia and apoplexy.The antistasis agents selected for cardiovascular and cerebrovascular diseases are classified according to the syndrome type,Xuefu Zhuyu pills and Peiyuan Tongnao capsule are selected as the special prescription of proprietary Chinese medicine,and the unreasonable use of drugs is analyzed and suggested,which provides a reference for the clinical rational application of antistasis agents.The use of respiratory system drugs is analyzed in the fifth chapter.The importance of expectorant,antitussive and antiasthma drugs and traditional Chinese patent medicine for removing phlegm and relieving cough and relieving asthma in the treatment of respiratory diseases is briefly described.The varieties and sales amount of respiratory drugs in the fourth quarter of 2018 are classified and compared by using Excel software,and the sales situation of antiasthma drugs and traditional Chinese patent drugs for removing phlegm and relieving asthma are analyzed.The drug use of respiratory system drugs is analyzed by using DDD analysis method and amount ranking method,and it is concluded that proprietary Chinese medicine has played an important role in respiratory diseases and provided reference for drug purchase and reserve.The research results are summarized in the sixth chapter of this paper,and some suggestions on clinical drug use and work are offered.
