| Objective:To investigate the curative efficacy of transurethral resection prostate(TURP)in patients with benign prostatic hyperplasia(BPH)and its correlation with preoperative age,duration of disease,body mass index(BMI),total prostate specific antigen(TPSA),total volume of prostate(TVP),intravesical prostatic protrusion(IPP),maximum detrusor pressure(Pdet.max),and Pdet.1.Methods:The investigator review the clinical datas of 113 patients with BPH who underwent TURP between October 2016 and June 2018 at the Zhongda hospital affiliated to the Southeast University,and record the international prostate symptom score(IPSS),quality of life score(QOL),and maximum urinary flow rate(Qmax).Observing symptom improvement(SI)(postoperativeIPSS/preoperativeIPSS)andfunctional improvement(FI)(postoperative Qmax-preoperative Qmax).SI≤0.5 was defined as an effective group(very good or good),SI>0.5 was defined as an invalid group(general or ineffective);FI≥5.0ml/s was defined as an effective group(very good or good),FI<5.0ml/s was defined as an invalid group(general or ineffective).Univariate analysis was performed for age,duration of disease,BMI,TPSA,TVP,IPP,Pdet.max and Pdet.1,and then investigator make multivariate logistic regression analysis.The correlation between Pdet.1et.1 and the improvement of IPSS urination symptom,storage symptom and Qmax was analyzed.Result:The mean IPSS of all patients decreased from 24.7 points to 8.5 points.There were 19cases in invalid group and 94 cases in effective group.There was significant difference in preoperative IPP and Pdet.max between the two groups(P<0.05).There were 23 cases in invalid group and 90 cases in effective group after operation.There were significant differences in the duration of disease,IPP and Pdet.maxet.max between the two groups(P<0.05).Univariate analysis showed that duration of disease,IPP and Pdet.max were important factors influencing postoperative efficacy.Multivariate analysis suggested that IPP and Pdet.maxet.max were independent factors.Conclusion:Preoperative IPP and Pdet.maxet.max of BPH patients treated with TURP were correlated with symptom functional efficacy.Duration of disease,IPP and Pdet.maxet.max were correlated with postoperative functional efficacy.IPP and Pdet.max are independent factors.Preoperative Pdet.1was related to the improvement of IPSS urinary storage symptoms.Preoperative urodynamics,prostate ultrasound and other auxiliary examinations are very important for predicting postoperative efficacy. |
