Objective: Serum 1,5-anhydroglucitol(1,5-AG)is a new glycaemic marker that reflects the control status and fluctuations of short-term glucose. However, its relationship with ?-cell function has rarely been reported, especially in people with normal glucose metabolism. This study aimed to investigate the correlation between 1,5-AG and ?-cell function in Chinese adults with different glucose metabolism statuses.Patients and measurements: A total of 2184 subjects with no prior history of diabetes participated in this study, all of whom underwent an oral glucose tolerance test(OGTT).For each participant, homeostasis model assessments for insulin resistance(HOMA-IR)and ? cells(HOMA-?)were calculated to reflect insulin resistance and ?-cell secretory function, respectively.Subjects were divided into three different glucose metabolism groups according to their OGTT.The correlations between 1,5-AG and both HOMA-IR and HOMA-? among the different groups of subjects were observed, and other possible interference factors were corrected.Stepwise linear regression analysis was performed to find the independent factors for HOMA-? and HOMA-IR.Results:Serum 1,5-AG levels were significantly different between the three groups.A significant positive correlation between 1,5-AG and HOMA-? was shown in the diabetesmellitus(DM)group(r = 0.265,p < 0.001)but not in the normal glucose tolerance(NGT)or impaired glucose regulation(IGR)group, and it still existed after adjusting for other indicators but disappeared after further correction for serum uric acid levels. In the DM group, both 1,5-AG and HbA1 c were independent factors for HOMA-?(1,5-AG: ? = 0.772,p = 0.023;HbA1c: ? =-7.52,p = 0.003).In addition, after using a stratified analysis,we found that 1,5-AG was still an independent factor for HOMA-? in the normal uric acid(NUA)group, but this was lost in the high uric acid group. Conclusion: 1,5-AG can reflect different glucose metabolism statuses. For those in the NUA group within the DM group,1,5-AG could be used as an auxiliary observation index reflecting the secretory function of ? cells.
