Objective: To observe the changes in serum level of Nesfatin-1 in patients with Type2 diabetic retinopathy of different degrees,and to analze the correlation between serun Nesfatin-1 and T2 DR.Methods: Choose from October 2018 to May 2019,a total of 187 T2 DM patients admitted to the Department of Endocrinology,Affiliated Hospital of ZunYi Medical University were recruited in this study and divided into 3 groups according to results of eye-ground photography,that is,75 patients in the simple T2 DM group(T2DM group),67 patients in the non-proliferative diabetic retinopathy(NPDR group)and 45 patients in the proliferative diabetic retinopathy(PDR group).Another 68 people with age and gender matching in the physical examination center were randomly selected as normal control group(NC group).General clinical data of the selected subjects were collected.Serum Nesfatin-1 levels were determined by enzyme linked immunosorbent assay(ELISA).Calculate the insulin resistance index and NLR.SPSS22.0 software was used for statistical analysis.One-way anova or kruskal-wallis H test were used for comparison among groups.Spearman analysis,and Logistic regression analysis were used to determine the relationship.P < 0.05 was considered statistically significant.Results: 1.The clinical data of NC,T2 DM,NPDR and PDR groups were compared:there were no statistically significant differences in age,sex and course of disease among the groups(P > 0.05).Compared with the NC group,BMI,NLR,TG,TC,LDL-c,HbA1 c,FPG,FIns,HOMA-IR was significantly increased and was HDL-c significantly decreased in T2 DM group(P < 0.05).Compared with the NC group,BMI,NLR,TG,TC,LDL-c,HbA1 c,FPG,FIns,HOMA-IR was significantly increased and hdl-c was significantly decreased in the NPDR group(P < 0.05).Compared with the NC group,BMI,NLR,TG,TC,LDL-c,HbA1 c,FPG,FIns,HOMA-IR wassignificantly increased and LDL-c was significantly decreased in the NPDR group(P< 0.05).Compared with the T2 DM group,NLR,TG,HbA1 c,FPG was significantly increased in the NPDR group(P < 0.05).Compared with the T2 DM group,NLR,TG,LDL-c,HbA1 c,FIns,HOMA-IR was significantly increased(P < 0.05).Compared with the NPDR group,NLR,FIns,HOMA-IR was significantly increased in the PDR group(P < 0.05).1.Comparision of serum Nesfatin-1 levels in each group,the serum level of Nesfatin-1in the T2 DM group was significantly lower than in the NC group,and the PDR group was significantly lower than in the T2 DM group and NPDR group.(P<0.05).2.Spearman correlation analysis showed that serum Nesfatin-1 was negatively correlated with BMI,TG,NLR,HbA1 c,FIns,HOMA-IR(r were-0.159,-0.146,-0.268,-0.149,-0.036,-0.449,all P<0.05).4.Logistic repression analysis showed that TG,HbA1 c was risks factors for T2 DM with DR,meanwhile Nesfatin-1 was protect factor for T2 DM with DR.Conclusion:Serum level of Nesfatin-1 were significantly reduced in patients with diabetic retinopathy and may be a protective factor for the development and progression of diabetic retinopathy.
