Objective: To analyze the influence of HPV infection on the curative effect and prognosis of hypopharyngeal carcinoma.Methods:A total of 40 cases of hypopharyngeal carcinoma were collected from August2013 to January 2019 in the Department of Otorhinolaryngology,the median age was 58 years old;smoking and drinking habits were classified into Group?,Group?,Group?;Type :pyriform fossa type(28 cases);retro-cricoid carcinomas type(10 cases);posterior pharyngeal wall type(2 cases);According to the TNM staging standard established by American Joint Committee on cancer(AJCC)in 2017,including 1 with T1,4 with T2,19withT3 and 16 with T4.There were 11 cases of N0,5 cases of N1,11 cases of N2 and 3cases of N3.There were 0 case in Stage?,2 cases in Stage ?,9 cases in Stage ? and 29 cases in Stage ?.The pathological diagnosis was all squamous-cell Carcinoma;the pathological differentiation was high differentiation in 12 cases,middle differentiation in20 cases,poorly differentiation in 8 cases;32 cases with lymph node metastasis,8 cases without lymph node metastasis;Six patients were treated with surgery alone,and 34 patients received postoperative radiotherapy at a dose of 50 ~ 66 Gy,with an average dose of 60 gy.Testing for HPV by fluorescence quantitative PCR technique.To analyze the relationship between HPV infection and incidence age,sex,smoking and alcohol history,type,stage,tumor differentiation,lymph node metastasis,treatment,TRG grade and prognosis in patients with hypopharyngeal carcinoma.Results: 1.In 40 hypopharyngeal carcinoma,HPV was positive in 6 cases(15%)and negative in 34 cases(85%).2.All of the 6 HPV positive patients with hypopharyngeal carcinoma had 1 or more high-risk HPV types.HPV59 predominated in high risk type,accounting for 50%,HPV43 in low risk type.3.There were significant differences between the HPV positive patients with hypopharyngeal Carcinoma and the HPV negative patientswith hypopharyngeal carcinoma in smoking(Group ? 17 cases,HPV + 6 cases;Group ?18 cases,HPV + 0 case;Group ? 5 cases,HPV + 0 case),drinking(Group?11 cases,HPV + 0 case;Group ? 15 cases,HPV + 2 cases;Group ? 11 cases,HPV + 2 cases;3non-drinkers,HPV+2 cases)TRG classification(grade 1a: 10 cases,HPV+ 5 cases;grade1b: 8 cases,HPV+ 2 cases,grade 2 to 3: 22 cases,HPV+ 0 case),tumor differentiation(high differentiation:12 cases,HPV + 0 case;middle differentiation:20 cases,HPV + 0case;poorly differentiation:8 cases,HPV+ 6 cases)The differences were statistically significant(P < 0.05).In patients with hypopharyngeal cancer,the survival time of the HPV positive group was(35.32±4.17)months longer than that of the HPV negative group(23.52±4.49)months,and the difference was statistically significant(P<0.05).Conclusion: 1?HPV infection and hypopharyngeal cancer have a certain correlation,the virus subtype is mainly high-risk HPV 59.2?HPV infection in patients with hypopharyngeal cancer may be associated with smoking,drinking and tumor differentiation.The infection rate of HPV in patients with drinking and smoking is low.3?The tumors of HPV positive patients were mostly poorly differentiation.4?HPV positive patients have a longer survival period.
