Objective:To investigate the the survival outcome of surgery and the prognostic factors affecting the results of treatment in patients with hypopharyngeal carcinoma.Methods:A retrospective analysis of 129 patients who received operation with hypopharyngeal cancer from March 2008 to March 2016 was performed.All clinical characteristics like age,gender,Charlsonscore,tumor site,smoking index,drinking,T-stage,N-stage,clinical stage,type of operation,pathological grade,type of pathology,surgical margin,type of neck dissetion,with or without second primary tumor as well as mode of treatment were included in the analysis.A life table method was employed to calculate the overall survival rate.The univariate analysis was carried out with Kaplan-Meier metod,while the multivariate analysis was performed with Cox proportional hazard model.Results:Of the 129 patients with hypopharyngeal cancer,the median survival time was53 months(0.3~106.4months).Overall 3and5 years survival rate was53%,38% respectively.46 cases were died and 13 cases were lost among the follow up duration.According toMTF score,116 cases recovered swallowing function completely,7cases in general and 6cases poorly.Tumor site,T-stage,N-stage,clinical stage,type of operation,mode of treatment were significant prognostic factors of hypopharangeal carcinoma.The multivariate analysis suggested that N-stage,clinical stage,mode of treatment were independent risk factors affecting prognosis.Conclusion: 1.N-stage,clinical stage,mode of treatment were important independent predictors of survival in patients with hypopharyngeal cancer.2.Induction chemotherapy before operation plus operation followed by chemoradiotherapy could improve the survival rate of hypopharyngeal cancer,especially advanced hypopharyngeal cancer.
