Objective:To explore the allergen distribution and clinical significance of fungal rhinosinusitisthed and the relationship between allergic factors and the prognosis of patients through the allergen detection and clinical analysis of fungal rhinosinusitis,so as to enhance the comprehending of fungal rhinosinusitis.Methods:74 cases of non-invasive fungal rhinosinusitis with data integrity(divided into two subgroups:fungal ball,allergic fungal rhinosinusitis)treated in the department of otorhinolaryngology of our hospital through surgical treatment from November 2017 to June 2019 were collected as the observation group,and 40 cases of chronic rhinosinusitis without nasal polyps without family history of allergic diseases and allergic rhinitis were collected as the control group.All patients were tested for allergens by the URANUS AE 85 automatic enzyme immunoassay instrument of Shenzhen Aikang Biotechnology Co.,Ltd.upon admission to hospital.All patients were treated with functional endoscopic sinus surgery(FESS)under general anesthesia with intubation.The observation group and the control group are treated strictly in accordance with the same treatment in the perioperative management.12 weeks after the operation to conduct Lund Kennedy scores by nasal endoscopy,follow-up 6 months to 12 months,collect the clinical data,applicate SPSS17.0 software for statistical analysis.Results:There were no statistically significant differences in age and gender between the observation group and the control group(P>0.05);The distribution of inhalant allergens sIgE in the observation group:The most common were House dust mite 6.76%(5 cases)and dermatophagoides farinae 6.76%(5 cases),followed by house dust 4.05%(3 cases),The distribution of the food allergens:The most common were peanut 4.05%(3 cases)and crab 4.05%(3 cases),followed by beef 2.70%(2 cases)and soybean 2.70%(2 cases).The positive rate of inhalant allergens was higher than that of food allergens,and the difference was statistically significant(P<0.05).The positive rate of TIgE in the observation group was higher than that in the control group,the differences was statistically significant(P<0.05);The positive rate of eosinophils in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).12 weeks after the operation,the value d of Lund Kennedy score of nasal endoscopic review in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05),the value d of Lund Kennedy score of nasal endoscopic review in AFRS group was lower than that in FB group,the difference was statistically significant(P<0.05).The effective rate of the observation group was 82.43%,the effective rate of the control group was 90.00%,the difference between the two groups was statistically significant(P<0.05).The effective rate of fungal ball was 85.71%and the effective rate of allergic fungal rhinosinusitis effective rate was 72.22%,the difference was statistically significant between the two group(P<0.05).Patients with fungal rhinosinusitis who had positive allergen had lower efficacy than those with negative allergen,the difference was statistically significant(P<0.05).Conclusion:1.The major allergens of fungal rhinosinusitis in Zunyi are inhalant allergens,followed by food allergens,the most common inhalant allergens are house dust mites and dermatophagoides farinae,and the most common food allergens are peanut and crab;2.Fungal rhinosinusitis are less effective than Chronic rhinosinusitis without nasal polyps,and the curative effect of Fungal ball is better in Fungal rhinosinusitis;3.Allergen-positive fungal rhinosinusitis patients with poor treatment results,allergic factors may influence the occurrence,development and postoperative recovery of fungal rhinosinusitis.
