Background:Sepsis is the most important cause of acute renal injury in the severe patients,the incidence of AKI in patients with sepsis is 41.5%~53.2%.In the ICU the sepsis is the most common cause of the disease.Sepsis AKI accounts for 32.40%~47.5% of all the AKI patients,and the mortality of the patient is significantly higher,which is 50.6%~ 70.2%.Survivors also had a significantly increased risk of chronic kidney disease(CKD).In addition,the incidence of adverse cerebrovascular events in the survivors was significantly higher than that of non-AKI patients,and the quality of life was significantly lower than that of non-AKI patients.AKI caused by sepsis in severe patients brings great economic burden to the society and patients' families.Objective:By collecting sepsis patients with acute kidney injury cases,the clinical data was analysed in a retrospective research.Discusses the value of strong ion gap(SIG)in predicted the severity of acute kidney injury in sepsis and prognosis judgement of diagnosis and treatment of acute renal injury of various offers.Methods:Choose between December 2017 and December 2019 in China-Japan union Hospital of Jilin University diagnosis and treatment of 166 patients with sepsis in the ICU(which is not associated with sex,79 cases of patients with acute kidney injury,with 87 patients with acute kidney injury)clinical data,set up without acute renal injury of sepsis patients as control group,with acute kidney injury in patients with sepsis as experimental group.28 cases of experimental group was divided into three groups: 1,2)30 cases,3 phase of 29 cases,diagnostic criteria in2012 to improve the prognosis of kidney disease worldwide organization(KDIGO)clinical practice guidelines grading criteria for the diagnosis of acute renal injury,except and other basic diseases that may influence the results,such as chronic renal insufficiency,other causes of shock(uncontrolled hemorrhagic shock,cardiac shock),etc.Observed between groups of SIG,acute physiology and chronic health evaluation SIG and(APACHE ?)score,and the correlation of hospitalization days,and adopt the receiver-operating characteristic curve(ROC)comparison SIG,serum creatinine(SCr),APACHE ? score predicts sepsis the effectiveness of the severity of acute kidney injury.Results1.There was no statistical difference in age and gender of the patients.2.Stage 2,3 and 2 groups,between the two groups in the level of the SIG,SCr,APACHE ? statistical difference between group.3.SIG levels and APACHE ? score(r = 0.567,P < 0.01),hospitalization days(r = 0.589,P < 0.01)were positively correlated.4.SIG in predicting 3 phase of the area under the ROC curve is superior to the APACHE ? score(0.902 + /-0.045 vs.0.658 + /-0.072,P <0.01),but with SCr(0.902 + /-0.045 vs.0.826 + /-0.054,P > 0.05)there was no significant difference is statistically significant.Conclusion1.The SIG value of sepsis patients with acute kidney injury was significantly higher than that of sepsis patients without acute kidney injury;2.SIG has a positive correlation with the severity of sepsis patients with acute kidney injury,which plays an important role in predicting the severity of sepsis patients with renal injury.3.SIG has a positive correlation with the adverse prognosis(mortality,length of stay)of patients with sepsis acute kidney injury,and it plays an important role in the assessment of prognosis.4.Sepsis in patients with acute kidney injury,SIG and SCr levels,APACHE ? score has significant positive correlation;And SIG APACHE ? score on kidney injury in the prediction of sepsis sex sensitivity is higher,but there was no sidnificant different between SIG and SCr.
