Objective:To observe whether dexmedetomidine as a ropivacaine adjuvant can effectively improve the analgesic effect of perioperative laparoscopic nephrectomy in patients with vertical spinal muscle block.Methods:A total of 90 patients who underwent laparoscopic nephrectomy in our hospital were selected as the research objects.They were divided into3 groups according to the random number table method,the control group(group P),the ropivacaine group(group R),and right you.Tomidin combined with ropivacaine group(DR group),30 cases in each group.Patients in the DR group and the D group were treated with conventional tracheal intubation under general anesthesia and underwent ultrasound-guided erector spinae plane block.The DR group was given 1?g / kg dexmedetomidine and 0.5% ropivacaine for 20 ml.0.5%ropivacaine 20 ml,no vertical spinal muscle block in group P.All three groups were given PCIA after surgery.Intraoperative hemodynamic changes and VAS scores of the three groups were recorded.The intraoperative analgesics of sufentanil,the number of effective compressions of the analgesic pump,and the number of remedialanalgesics were recorded.Postoperative adverse reactions were recorded in the three groups of patients.Results:The general condition of the patient,the length of the operation and the amount of intraoperative blood loss were not statistically significant(p> 0.05);compared with group P,the changes in heart rate and average arterial pressure before and after surgery in group R and DR were small,The difference was statistically significant(p <0.05);compared with the P group and the R group,the use of intraoperative analgesic drug sufentanil in the DR group was significantly reduced(p <0.05);The 48-hour VAS score was significantly lower in the P and R groups(p <0.05);the number of analgesic pump compressions in the DR group was significantly lower in the P and R groups(p <0.05);compared with the P and R groups,The incidence of adverse reactions in the DR group was significantly reduced(p <0.05).Conclusion:Dexmedetomidine combined with ropivacaine for ultrasound-guided erector spinae muscle block can provide a more complete analgesic effect during laparoscopic nephrectomy,and effectively reduce the amount of opioid analgesic drugs and reduce The incidence of adverse reactions,analgesia effect of patients after surgery is better,and it is worth further clinical use.
