ObjectiveOur goal was to explore the clinical effect of joint localization of femur and acetabulum in primary cementless total hip replacement.MethodsFrom December 2017 to March 2019,110 patients with aseptic necrosis of the femoral head and osteoarthritis of the hip to undergo total hip arthroplasty were selected.All patients were primary unilateral total hip replacement,a total of 110 hips,49 males and 61 females,with an average age of 61.9 years.Clinical diagnosis: 69 cases of aseptic necrosis of the femoral head,41 cases of osteoarthritis of the hip.The patients were randomly divided into two groups according to the way of operation,control group: 52 hips were implanted with acetabulum cup prosthesis by traditional operation method;experimental group: 58 hips were operated by combined anteversion technique.Posterolateral approach was used in both groups,and the operation was performed by the same senior surgeon.There was no significant difference in general clinical data between the two groups(P > 0.05).Harris score,postoperative imaging comparison and dislocation were analyzed.SPSS 22.0 system software was used,the counting data was expressed by the number of cases,the comparison between groups was expressed by ?~2test,the measurement data was expressed by (?),and the comparison between groups was expressed by t test,P < 0.05,which was statistically significant.ResultsThe Harris scores of the control group and the experimental group were(82.1 ± 7.9),(85.6 ± 7.4)6 months after operation,12 months after operation,Harris scores were(85.9 ± 6.4)and(88.5 ± 6.5).There was significant difference between the two groups in the follow-up score of 1 year(P < 0.05).Postoperative imaging comparison: the adverse rate of anteversion was 28.85%(15 / 52)in the control group and 3.45%(2 / 58)in the experimental group.There was significant difference between the two groups(P < 0.05).Single factor analysis of the influence of combined anteversion on postoperative dislocation showed that there was a correlation between dislocation and the range of combined anteversion(P < 0.05).ConclusionThere is a great variation in femoral anteversion,In the operation with biological prosthesis,only 71.15% of the combined Anteversion meets Dorr safety range with traditional surgical methods.The combination of anteversion technology can improve the accuracy of prosthesis implantation.The function of hip joint can be improved and the incidence of dislocation can be reduced.
