Status And Influential Factors Of Sleep In Children With Adenoid Hypertrophy And The Relationship With Serum ESM-1

Objective:To understand the prevalence and influencing factors of sleep problems in children with adenoid hypertrophy and explore the association of adenoid hypertrophy and sleep problem with serum EAM-1.Method:The observation group included 135 patients,aged 4 through 12 years,diagnosed with adenoid hypertrophy in the ear,nose,and throat(ENT)of the Second hospital of Jilin University.And 135 normal children with similar age and sex,examined in the pediatric medical center,were selected as the normal control group.The Chinese version of Children's Sleep Habits Questionnaire was used to investigate the sleep problems of the two groups.According to CSHQ's score,the observation group were divided into the case group(children with sleep problems)and the observation-control group(children without sleep problems).30 children were selected randomly in the case group,the observation-control group and the healthy-control group(children in the normal control group without sleep problems)respectively.The levels of ESM-1 were measured by double-antibody sandwich ELISA in the three groups.Result:The survey showed that the sleep problem detection rate was 74.8%(101/135)in children with adenoid hypertrophy compared with 46.7%(63/135)in the normal children.The difference was significantly statistical significant(P<0.001).The total scores of CSHQ,bedtime resistance,sleep duration,sleep anxiety,parasomnias,sleep disordered breathing,daytime sleepiness in the observation group were significantly higher than those in the normal control group(all P<0.05).There are statistically significant differences in the adenoid hypertrophy pathogenesis,the size of adenoids,parental anxiety,living with parents,BMI classification,maternal education,family education style and mode of delivery between the case group and the observation-control group(all P<0.05).Logistic regression analysis showed that long adenoid hypertrophy pathogenesis,highly degree of adenoid hypertrophy grading,parental anxiety,living with parents,overweight or obesity,cesarean delivery,critical family education were risk factors of children with adenoid hypertrophy combined with sleep problems,and the high maternal education were protective factors.The levels of serum ESM-1 in the case group was significantly higher than those in the observationcontrol group and in the healthy-control group(all P<0.05).The levels of serum ESM-1 in the observation-control group was significantly higher than those in the healthy-normal group(P<0.05).No significantly statistical differences were found in the levels of serum ESM-1 among children with different adenoid hypertrophy grading(all P>0.05).There was a negative correlation between the levels of serum ESM-1 and adenoid hypertrophy pathogenesis in the case group(rho=-0.476,P<0.05),but no correlation was found in the observation-control group(rho=-0.336,P>0.05).No significantly statistical differences were found in the levels of serum ESM-1 among different BMI classification children in the case group,the observation-control group and the healthy-control group(all P>0.05).Conclusion:1.The sleep problem detection rate was 74.8% in children with adenoid hypertrophy,and its significantly higher than the normal control group2.The adenoid hypertrophy pathogenesis,the size of adenoids,parental anxiety,living with parents,BMI classification,the level of maternal education,family education style and mode of delivery influence the sleep health of children with adenoid hypertrophy.And the long adenoid hypertrophy pathogenesis,high degree of adenoid hypertrophy grading,parental anxiety,living with parents,overweight or obesity,cesarean delivery,critical family education are risk factors,the highly maternal education are protective factor.3.A significantly positive correlation was found between the levels of serum ESM-1 and adenoid hypertrophy and adenoid hypertrophy with sleep problems.
