Objective:To study the clinical characteristics and prognosis of solid tumors patients with bone marrow metastasis,and to analyze the clinical characteristics,treatment outcome and prognosis of breast tumors patients with bone marrow metastasis.Method:From October 2010 to October 2018,145 adult solid tumors patients with bone marrow metastasis in the First Hospital of Jilin University and 36 patients with advanced breast cancer who underwent bone marrow smears and biopsies for blood cell reduction during the same period were collected.The basic information of the patient includes the clinical characteristics of the patients,the location of the metastasis,the previous treatment,the blood condition,the follow-up treatment and the survival time.To analyze the characteristics on epidemiological,clinical characteristics and prognosis of bone marrow metastasis in patients with solid tumors,focusing on the clinical characteristics,treatment outcomes and prognosis of bone marrow metastases in breast cancer patients.SPSS 25.0 software was used for statistical analysis,and?~2 test was used for comparison between groups.Multivariate Logistic analysis was used to analyze related factors.The time of OS were expressed as median(95%CI),Log-Rank test was performed by Kaplan-Meier method,and COX proportional hazard model was used for multivariate analysis of prognostic survival.All datas were statistically significant with P<0.05..Results:1.Among the 145 patients of adult solid tumors with bone marrow metastases in our center,106(73.1%)patients clearly found the primary tumor,and 39(26.9%)patients did not find the primary tumor.Among the patients with definite primary tumors,70 cases(66%)were breast cancer,11 cases(10.4%)were prostate cancer,10cases(9.4%)were lung cancer,7 cases(6.7%)were gastric cancer,and 8 cases(7.5%)were other types.2.The clinical features of bone marrow metastasis in patients with solid tumors are bone metastases and anemia.Anemia occurred in 89.7%of patients,and 92.5%of patients with bone marrow metastases with clear primary tumors combined bone metastases.3.The median OS of bone marrow metastasis in patients with solid tumors was4.9 months,the median OS of patients with unknown primary tumors was 2.95months,and the median OS for breast cancer was 5.93 months,the median OS for prostate cancer was 9.47 months,the median OS for lung cancer was 0.83 months,the median OS for gastric cancer was 4.67 months,and the median OS for other tumor types was 2.1 months.4.The treatment has an impact on the prognosis of bone marrow metastasis in patients with solid tumors.The median survival of patients with anti-tumor therapy is significantly better than that of the untreated group(8.7 verus 1.9 months,P<0.001).The median OS was better in the treated group than in the untreated group(11 verus2.06 months,P<0.001),and the median OS of chemotherapy patients was longer than that of endocrine therapy and untreated patients(12.23 verus 8.47 verus 2.07months,P<0.001)..Conclusions:1.The incidence of bone marrow metastasis in breast cancer patients is significantly higher than that in prostate cancer,lung cancer,gastric cancer and other tumors.2.The presence of bone metastases and anemia in clinical features highly suggests the possibility of bone marrow metastases and is an indication of bone marrow aspiration and biopsy.3.There is a close relationship between bone metastases and bone marrow metastases.The rate of bone marrow metastases in patients without bone metastases is extremely low.4.Chemotherapy can improve the time of overall survival in patients with breast cancer and bone marrow metastases.
