| Objective:The clinical data of patients with gastric polyps were retrospectively analyzed to explore the clinical characteristics of these patients,to improve the understanding of common gastric epithelial polyps,and to provide reference for clinicians’ diagnosis and treatment gastric polyps.Methods:A retrospective analysis was performed on 475 patients who were hospitalized in the First Hospital of Jilin University from January 2018 to October 2019,who were diagnosed with gastric polyps.The patient’s gender,age,endoscopic manifestations,pathological types of polyps,atypical hyperplasia and blood lipid levels were recorded in detail to summarize the general clinical characteristics of patients with gastric polyps.In order to discuss the clinical characteristics and cancerous status of patients with gastric adenomatous polyps in detail,147 patients who were diagnosed as gastric adenomatous polyps by gastroscopy and pathological biopsy from January 2009 to October 2019 were retrospectively analyzed again.The study recorded the size,location,number,histological type and canceration of gastric adenomatous polyps to summarizes the clinical characteristics and canceration of patients with gastric adenomatous polyps.Results:1.The proportion of female patients with gastric polyps was higher than that of male patients(65.05%>34.95%).The proportion of patients with gastric polyps in the young and middle-aged group was higher than that in the elderly(59.37%> 40.63%).Compared with gastric polyps in the elderly group,the proportion of women in the young and middle-aged group was higher,and the proportion of men was lower(P = 0.025).2.The proportion of patients with multiple gastric polyps was higher than that of single patients.Compared with male patients with gastric polyps,female patients with gastric polyps had a higher proportion of multiple polyps and a lower proportion of single polyps(P = 0.000).3.The distribution of gastric polyps in descending order was 52.42% of gastric body,34.31% of gastric fundus,8.84% of gastric antrum,2.74% of gastric cardia,1.05% of gastric body and gastric antrum junction,0.42% of pylorus and 0.21% of stomach angle.The proportion of gastric body polyps was highest.The most common size of gastric polyps was diameter no more than 1cm(87.58%),followed by diameter 1.1-2.0cm(10.32%)and diameter more than 2.0cm(2.11%).4.The most common gastric polyps were fundus glandular polyps(FGPs)(83.79%),followed by hyperplastic polyps(HPs)(14.11%)and adenomatous polyps(APs)(2.11%).Compared with fundus glandular polyps,the proportion of hyperplastic and adenomatous polyps with diameter of 1.1-2.0 cm and more than 2.0 cm was higher,and the proportion of no more than1.0 cm was lower(P = 0.000,P = 0.000).Compared with fundus glandular polyps,the proportion of atypical hyperplasia of hyperplastic polyps and adenoma polyps was higher(P = 0.000,P = 0.000).Adenomatous polyps accounted for a higher proportion of atypical hyperplasia than hyperplastic polyps(P = 0.000).The surface color of hyperplastic polyps and adenoma polyps was redder than that of fundus glandular polyps(P = 0.000,P = 0.000).5.The proportion of gastric mucosa with erosion in fundus glandular polyps was higher than that in hyperplastic polyps(P=0.000).The proportion of gastric mucosa with erosion in elderly patients with gastric polyps was higher than that in young patients(P =0.003).Compared with fundus glandular polyps,the proportion of hyperplastic polyps with gastric xanthoma was higher(P = 0.012).6.Female patients with gastric polyps had higher rates of elevated cholesterol and elevated low-density lipoprotein cholesterol than male patients(P = 0.000,P = 0.009).7.The proportion of male gastric adenoma patients was higher than that of females(60.54%> 39.46%).The proportion of patients with gastric adenoma in the elderly group(≥60 years)was higher than that in the young and middle-aged group(<60 years)(65.30%> 34.69%).The proportion of single gastric adenomatous polyps was significantly higher than multiple gastric adenomatous polyps(81.63%> 18.36%).8.The distribution of gastric adenomas in descending order was 44.22% of gastric antrum,27.89% of gastric body,8.16% of gastric cardia,4.76% of gastric body and gastric antrum junction,3.40% of gastric fundus,10.20% of gastric angle and 1.36% of pylorus.9.Among the gastric adenoma patients,tubular adenoma was the most(66.67%),villous tubular adenoma was the second(30.61%),and villous adenoma was the least(2.72%).The highest rate of canceration was villous adenoma(50%),followed by villous tubular adenoma(17.77%),and the lowest rate was tubular adenoma(3.06%).The canceration rate of villous tubular adenoma and villous adenoma was higher than that of tubular adenoma,and the comparison between the two groups was statistically significant(P =0.000,P =0.011).The canceration rate of adenoma with diameter more than 2cm was 20.51%,and that with diameter no more than 2cm was 4.63%.The canceration rate of adenoma with diameter more than 2cm was higher than that with diameter no more than 2cm(P=0.006).The canceration rate of gastric adenoma was no significant difference among groups with different sex,age,number of occurrence and distribution sites.10.The resection methods of gastric adenoma were ESD(55.36%),followed by EMR(32.14%),biopsy forceps(8.93%)and APC(3.57%).Conclusions:1.The overall distribution of patients with gastric polyps is mainly female,gastric polyps are more common in the young and middle-aged.2.The most common pathological types of gastric polyps are fundus glandular polyps,followed by gastric hyperplastic polyps and gastric adenomatous polyps.The most common distribution of gastric polyps is the gastric body.The most common size of gastric polyps was diameter no more than 1cm.3.Female with gastric polyps are more likely to have elevated cholesterol levels and low-density lipoprotein cholesterol levels.4.The patients of gastric adenomatous polyps are mainly males and are mainly found in the elderly,The most common location is the gastric antrum,the most common pathological type of gastric adenomatous polyps is tubular adenoma,diameter more than 2cm,gastric villous tubular adenoma and gastric villous adenoma are prone to cancer. |
