Objective: To investigate the efficacy and safety of dapagliflozin in the treatment of chronic heart failure,and whether the patients can reduce the cardiovascular mortality in patients with heart failure.Besides,we conduct a systematic evaluation.Methods: Two Investigators independently used computer to search Cochrane Central Register of Controlled Trials,PubMed,Embase,The Cochrane Library,CNKI,Wanfang data and VIP databases to collect randomized controlled trials of dapagliflozin in the treatment of chronic heart failure.The search time limit is from the establishment of the database to February 1,2020.Screening the literature,extracting the data,sorting out the general characteristics included in the study and evaluating the bias risk being completed,the Revman5.3 software was used for Meta analysis.Results: A total of 7314 patients were included in 4studies.Effectiveness: Hospitalization for heart failure [OR=0.69,95%CI(0.59-0.80),P<0.00001];Cardiovascular mortality[OR=0.84,95%CI(0.72-0.99),P=0.04];Composite endpoint(hospitalization for heart failure complicated with cardiovascular death)[OR=0.78,95%CI(0.71-0.86),P<0.00001].MACE [OR=0.48,95%CI(0.09-2.37),P=0.37].Safety:Capacity exhaustion [OR=1.12,95%CI(0.90-1.38),P=0.30],Severehypoglycemia [OR=0.71,95%CI(0.24-2.13),P=0.54],Renal damage[OR=0.92,95%CI(0.74-1.15),P=0.47].Conclusion: Dapagliflozin can significantly reduce the admission rate of heart failure and cardiovascular death in patients with heart failure,and there are no obvious adverse events.However,due to the limited number of studies and the large difference in research time,More high-quality clinical studies and real-world use are needed to validate the results.
