Objective:This study aims to find out the clinical characteristics and in-hospital prognosis of myocardial infarction,specifically STEMI patients with hyperuricemia.Background:Cardiovascular disease comprises the most prevalent severe disorders in developed nations and is a rapidly growing problem in developing countries.Cardiovascular disease is becoming a public health issue worldwide as the burden of CVD increases.Acute myocardial infarction(AMI)is one of the most frequent diagnoses in hospitalized patients.Following 15 years in China,there will be a massive number of myocardial infarction patients as increasing risk factors and the aging population.Uric acid is a weak organic acid,and it is presented as an end-product of purine nucleotides degradation.In the human body,uric acid eliminates free oxygen radicals and is also an indicator of oxidative stress related to increased xanthine oxidase enzyme activity.The association between serum uric acid level and mortality of STEMI has been studied,but the study about the in-hospital prognosis of STEMI patients with hyperuricemia is few.Methods:The patients hospitalized in the cardiology department in Bethune's first hospital of Jilin University from January 1 to June 30 in 2017 were selected,all of whom met STEMI standard and accepted emergency surgeries and implanted stents.We assessed 548 patients with a diagnosis of myocardial infarction.After evaluating all the demographical,clinical,past medical,and investigation(Troponin I,Uric Acid,ECG,coronary angiography)records,all 548 patients were included for further analysis.Their information was complete.Their clinical information,vital signs,auxiliary lesions,number of stents,the prognosis of hospitalization were compared.Results:(1)There were statistically significant differences in sex among normal uric acid groups and the hyperuricemia group(P<0.001).There were no statistically significant differences in age,chronic myocardial infarction,high blood pressure,diabetes,and atrial fibrillation(P>0.05).We also carried out a subgroup analysis based on gender.In the hyperuricemia group,A higher proportion of patients were male,and a higher proportion of female patients have hypertension.(2)Myocardial infarction areas were compared among the two groups,and it was found that there were no statistically significant differences in anterior wall myocardial infarction,extensive anterior wall myocardial infarction,and inferior wall myocardial infarction(P>0.05).(3)There were statistically significant differences in hemoglobin,HDL,creatinine among normal uric acid groups and the hyperuricemia group(P>0.05),compared with acute STEMI patients with normal uric acid,and STEMI patients who had hyperuricemia have higer HDL?HGB and Creatinine level.There were no statistically significant differences in TG,WBC,and the total hospitalization days among the two groups.(P>0.05).(4)When coronary artery lesions were compared among the two groups,it was found that there were no statistically differences in single vascular lesion,double vascular lesions and triple vascular lesions(P>0.05).We also carried out a subgroup analysis based on gender.In the hyperuricemia group,A higher proportion of female patients have double vascular lesions and triple branch lesions.(5)When the criminal blood vessels were compared among the two groups,the differences were no significant(P>0.05).(7)Killip classification was found to statistically highly significant with p-value 0.003.From comparison among the groups of Killip classification,there is Killip 1 has 137 patients with high uric acid levels.In this group,compared with acute STEMI patients who had hyperuricemia,and STEMI patients with normal uric acid have lower Killip grades.(8)When the clinical prognosis during the hospitalization were compared,it was found that the proportion of ventricular tachycardia/ventricular fibrillation,pneumonia,cerebral hemorrhage,gastrointestinal bleeding,heart failure and death,and there was no statistically significant difference(P>0.05).Conclusion:(1)There was a statistically significant difference in sex among the normal uric acid group and the hyperuricemia group.The proportion of males in the hyperuricemia group is high.(2)Compared with acute STEMI patients who had normal uric acid,and STEMI,patients with hyperuricemia have higher Killip grades.(3)When the clinical prognosis during the hospitalization were compared,it was found that the proportion of ventricular tachycardia/ventricular fibrillation,pneumonia,cerebral hemorrhage,gastrointestinal bleeding,heart failure and death,and there was no statistically significant difference.(4)We carried out a subgroup analysis based on gender.In the hyperuricemia group,A higher proportion of female patients have double vascular lesions and triple branch lesions.
