Objective:Through comparativing the differences of the clinical features between elderly onset rheumatoid arthritis(EORA)and non-elderly onset rheumatoid arthritis(NEORA),to improve the ability to recognize EORA and offer a reference for clinical.Methods:792 cases of hospital patients with rheumatoid arthritis in NingXia Medical University General Hospital from December,2009 to December,2018 were collected,and divided into two groups according to the age of onset:two groups are EORA(?60 years,disease onset after60 years of age),and NEORA(<60 years,disease onset before 60 years of age).All patients are analyzed and compared based on the clinical features,such as general condition,comorbidities,complication,joint performance and laboratory tests.Using statistical methods to evaluate and make a conclusion.Results:1.General condition:(1)792 cases of hospital patients,680 were in the group of NEORA,112 were in the group of EORA.The highest frequency of onset age is 40-60 years old[459cases(58.00%)];(2)The proportion of male patients in two groups was higher than female,The female-to-male ratio is 4.27:1 in NEORA,1.29:1 in EORA(P<0.05);(3)The proportion of patients with smoking history in EORA was higher than that in NEORA[85 cases(12.8%)in NEORA?39 cases(34.8%)in EORA,(P<0.05)];(4)The proportion of patients with drinking history in EORA was higher than that in NEORA[71 cases in NEORA?21 cases(18.8%)in EORA,(P<0.05)].2.Comorbidities:(1)The proportion of patients companied with hypertension in EORA was higher than that in NEORA[122 cases(17.9%)in NEORA?45 cases(40.2%)in EORA,(P<0.05)];(2)The proportion of patients companied with diabetes in EORA was higher than that in NEORA[46 cases(6.8%)in NEORA,16 cases(14.3%)in EORA,P<0.05];(3)The proportion of patients companied with coronary heart disease in EORA was higher than that in NEORA[23cases(3.4%)in NEORA,12 cases(10.7%)in EORA,P<0.05].3.Complication:The proportion of patients companied with pulmonary interstitial change in EORA was higher than that in NEORA[150 cases(22.1%)in NEORA?62 cases(55.4%)in EORA,(P<0.05)].4.Joint performance:(1)There is no significant statistical difference in the value of affected jointscounts between EORA and NEORA(P>0.05);(2)The proportion of patients with involved ankle joint in EORA was lower than that in NEORA[374 cases(55.0%)in NEORA?47cases(42.0%)in EORA,(P<0.05)]?The proportion of patients with involved metatarsophalangeal joint in EORA was lower than that in NEORA[275 cases(40.4%)in NEORA?31cases(27.7%)in EORA,(P<0.05)],other involved joints(elbow,knee,wrist,metacarpophalangeal joint,proximal phalangeal joint)differences between EORA and NEORA is not statistically significant(P>0.05);(3)The first onset of large and small joints in EORA group was less than that in NEORA group(P>0.05);the first onset of large joints in EORA group was more than that in NEORA group(P>0.05);the first onset of small joints between the group of EORA and NEORA is not statistically significant(P>0.05).(4)The X-ray stagings about hand joints between EORA and NEORA are not statistically significant(P>0.05).5.Laboratory tests:The average level of ALB in NEORA was higher than that in EORA[34.70±4.62g/L in NEORA?33.28±5.20g/L in EORA,P<0.05)];(2)The average level of WBC in EORA was higher than that in NEORA[6.14(5.03,7.46)×10~9/L in NEORA?6.70(5.38,7.95)×10~9/L in EORA,P<0.05)];(3)There is no significant statistical difference in the level of Hb?PLT?PCT?RF?IgG?IgA?C3?C4?AKA?ESR?CRP between EORA and NEORA(P>0.05);(3)The level of IgM in NEORA was higher than that in EORA[1.33(0.95,1.79)g/L in NEORA?1.07(0.71,1.48)g/L in EORA(P<0.05)].(4)The positive proportion of anti-cyclic citrullinated peptide(anti-CCP)antibodies in NEORA was higher than that in EORA(P<0.05).Conclusion:1.There are more females than males in patients with EORA.2.In EORA,pulmonary interstitial changes are more common than those in NEORA group.3.Compared with NEORA,there were more large joints in the first onset joint.4.The level of IgM and the positive proportion of anti-CCP antibodies in EORA group were lower than those in NEORA group.
