Objective:Cone-beam CT(CBCT)was used to analyze and summarize the root morphology and anatomy of maxillary third molars,so as to provide some references for clinical autologous tooth transplantation and postoperative root canal treatment.Methods:CBCT data taken from may,2017 to may,2019 were randomly selected from the department of oral imaging,Bethune hospital,Shanxi Province,and all the patients gave informed consent to select the inclusion conditions that met the inclusion criteria:(1)The imaging morphology of the maxillary third molar was complete;(2)Root curvature is less than 10;(3)Complete root development;(4)No internal and external absorption of calcification;(5)No periodontal disease;(6)The CBCT image is clearly discernible and has no artifact;(7)Ages 18 to 65.Exclusion criteria: a history of root canal treatment or repair;Maxillary teeth after the metal lead to serious artifact of third molar.A total of 201 CBCT data containing 339 maxillary third molars were selected,and the data entry software was used to record the number of root and the number of root canals in 8positions.The relationship between the number of root canal system and the sex of tooth position was analyzed to guide the use of maxillary third molars as donor teeth in clinical autologous tooth transplantation.Results:Among the 201 CBCT images,there were 339 cases of maxillary third molars,including 84 males,117 females,172 on the left and 167 on the right,among which159(46.9%)had fused roots,105(31.0%)had double roots,73(21.5%)had three types,and only 2 of the four types.Fusion roots were 76.7%common in single canal;The double root type was mostly double root canal,accounting for 64.8%;Three are three main canals up to 97.3%;All four types have four canals.According to Vertucci classification,the fusion root medium type?accounted for 55.9%,the buccal root palatal root medium type?accounted for 47.6%,70.9%,the palatal root medium type?accounted for 69.9%,95.9%,98.6%,and the four root types were all types?.Type?(47.2%),?(19.4%)and type?(13.9%)were the main types of root canal in male fused roots.The root canal types of female fused roots were mainly type?(58.9%),type?(11.2%)and type ?(11.2%).Most of the root canal systems not included in Vertucci classification were C-shaped root canals,and 12 cases were fused roots,among which 7 cases were C1-type(58.3%),3 cases were C2-type(25.0%),and 2 cases were C3-type(16.7%).C-shaped root canals accounted for 3.5% of the maxillary third molars and 7.5% of the fused root of the maxillary third molars.According to the statistics,there was a correlation between the number of root and the number of root canals in the maxillary third molars(p<0.05).There was no statistically significant difference in the type of root canals in the maxillary third molar root canals on the left and right sides(p>0.05),and there was no statistically significant difference in the type of root canals in males and females(p>0.05).Conclusion:There were 77.9% maxillary third molar with fused root,buccal and palatal roots,and there was no statistical difference between left and right,gender,root number and canal type.The root number of maxillary wisdom teeth was significantly correlated with root canal number.Although there are many kinds of root canal system,most of them are multiple simple types,they are mainly type I.For the autograft of maxillary third molar,the clinical stomatologist must first be familiar with the anatomy of CBCT maxillary wisdom tooth.
