Objective: This paper reports the case of maxillary molars that presented two root canals in the Mesiobuccal root.To study the use of Twisted Files Ni-Ti Rotary instruments on clinical diagnoses and treatment of MB2 in the mesiobuccal root canal system of maxillary molarMethods: Collect ten clinical cases, take preoperative radiographs, and analyze the ten cases of disease, Standard straight-on and distal angle radiographs were taken of each tooth. A second canal was suspected if multiple periodontal ligament (PDL) spaces or apices were noted or if an instrument appeared to be off center in the root on a distal projection radiograph. Access preparations were routinely modified to a rhomboidal shape . Remove all the caries and leaky restoration,clear the pulp chamber,locate the root canal orifice,gain streight line access,use a stainless steel file an exploratory curve to creat a patent pathway. Probing and/or troughing of the fissure or groove between the mesiobuccal and palatal canals was performed . This procedure was conducted using irrigation . If an MB2 orifice was located, attempts were made to negotiate the canal with SybronEndo Pathfinder CS K1/K2files and Triple-Flex files. The troughing process was continued until the presence of the MB2 was ruled in or out.All working lengths were confirmed radiographically and by Root ZX. Using effective irrigants such as 2%chlorhexidine.Then the crow-down technique could be employed using TF Ni-Ti Rotary instruments in conjunction with the antimicrobial irrigation solution. filling root canal firmly .Take posto- perative radiographs, record the incidence rate of the mesiobuccal root canal system of maxillary molars and clinical feature, and evaluate the therapeutic effect. To be included and recorded as a second mesiobuccal canal, the canal had to be negotiable and obturated to its own apex or within 5 mm of the apex when contiguous with the first mesiobuccal canal.Results: In the ten clinical cases, there were all cases have the MB2 canal and all canals are filled. The occurrence of the MB2 canal in maxillary molars, depending on which method was used. The literature shows both clinical and laboratory results and that more MB2 canals can be found in the laboratory than were found clinically . The MB2 root canal of maxillary molar for lip of dentin, dentin debris and pulp diffuse calcification and pulp stone, make its clinical detection rate is low and orifice locating and getting through the difficulties.The mesiobuccal root canal of maxillary molar system anatomical configuration of various types, complex, clinical diagnosis and treatment are more difficult than routine cases .With the preoperative radiographs, a rhomboidal access, probing and/or troughing of the fissure or groove between the mesiobuccal and palatal canals,and using SybronEndo Pathfinder CS K1/K2 files, can increase the success rate of root canal therapy in maxillary molars. And using TF Ni-Ti Rotary instruments in conjunction with the irrigation by 2 %chlorhexidine filling root canal firmly. Maxillary molar can obtain good effect of root canal therapy. |
