Research On The Optimization Of Medical Payment Method Reform Under The Background Of National Medical Insurance

Recent years,the medical insurance system in China has been improving continuously,so almost all Chinese are covered by medical insurance.Therefore,representing patients,medical insurance institution has become the purchaser of medical service,which has advantage in negotiating with the providers of medical service.Medical payment methods enable medical insurance to function completely,so they greatly impact medical institutions.The reform of medical payment methods is progressing and improving in practice.As traditional medical payment methods cannot satisfy the needs of medical insurance due to its shortages,Chinese medical insurance is constantly reforming and improving in practice.A County is located in the southeast of Guangxi Zhuang Autonomous Region,covering an area of 2998 square kilometer.In 2009,a total of 1,220,000 people has covered by medical insurance for urban and rural residents and for urban workers,not counting migrant workers,so the coverage of medical insurance is more than 95%.It means that in A county,the universal medical insurance has been set up.However,poor access and high fee are still the two major problems in A county.As a third party which purchases medical service,the medical insurance institution takes the responsibility to promote medical reform.Medical payment method is important to promote the medical reform.The paper introduces the medical payment method used by the medical insurance institution in A County of Guangxi Zhuang Autonomous Region,and analyzes the shortages of original payment system,as well as the advantages and disadvantages of the reformed methods.Under the background of national medical insurance and incentive theory,it analyzes the problems exiting in the reform of medical payment methods in A county,and points out the the reasons are the roughpayment methods' not conforming to the complex medical service ways,deviation of incentive and restraint mechanism and the incomplete supporting system.Hence,the solutions targeting to the problems are changing the single-type payment method to compound methods,establishing the excitation and constraint mechanism which is compatible to the compound methods and related supporting system,so as to promote a further reform with excitation and constraint mechanism.
